1990-2015年湖北省交通伤害疾病负担研究

Research on disease burden caused by traffic injury in Hubei, 1990-2015

  • 摘要: 目的 分析1990-2015年湖北省交通伤害的疾病负担变化情况,为交通伤害防控提供参考依据。方法 利用2015年全球疾病负担研究的中国分省结果,采用死亡率、早死损失寿命年(YLLs)、伤残损失寿命年(YLDs)以及伤残调整寿命年(DALYs)作为测量疾病负担的指标,研究1990-2015年湖北省交通伤害导致的死亡及疾病负担变化情况。结果 2015年湖北省交通伤害死亡率为25.6/10万,男性和女性死亡率分别为37.7/10万和12.7/10万。1990-2015年,标化死亡率由25.3/10万降为22.1/10万,下降12.6%,男性和女性标化死亡率均下降。2015年湖北省交通伤害造成的DALYs、YLDs、YLLs分别为65.1、8.7、56.4万人年,与1990年相比,交通伤害总体DALY标化率由1 271.7/10万降至1 064.0/10万,降低16.3%,男性和女性分别降低8.3%和32.9%。2015年不同类型交通伤害中,步行道路伤害导致的DALYs最高,占53.4%。湖北省交通伤害所致DALYs主要分布在15~69岁年龄组。结论 与1990年相比,2015年湖北省交通伤害的标化死亡率和DALY率均下降,但造成的死亡和DALYs依然较高,尤其是步行道路伤害,男性交通伤害疾病负担远高于女性。

     

    Abstract: Objective To understand the disease burden caused by injury in the population of Hubei province from 1990 to 2015 and provide scientific evidence for the prevention and control of traffic injury. Methods Based on the results of the Global Burden of Disease Study 2015 (GBD 2015), the mortality, years of life lost due to premature mortality (YLLs), years lived with disability (YLDs) and disability-adjusted life years (DALYs) in Hubei of China were used to analyze the change of disease burden caused by injury in Hubei from 1990 to 2015. Results The mortality rate of traffic injury was 25.6/100 000 in Hubei in 2015 (37.7/100 000 in males and 12.7/100 000 in females). The standardized mortality rate decreased from 25.3/100 000 in 1990 to 22.1/100 000 in 2015, a decline of 12.6%. The standardized mortality rate decreased in both males and females. The DALYs, YLDs and YLLs by traffic injury were 651, 87 and 564 thousands person years respectively. The overall standardized DALY rate decreased from 1 271.7/100 000 in 1990 to 1 064.0/100 000 in 2015, a decline of 16.3%. The decrease rate was 8.3% in males and 32.9% in females. In different type traffic injuries in 2015, the DALYs caused by pedestrian walkway injury was highest, accounting for 53.4%. In Hubei, the DALYs caused by traffic injury were mainly distributed in age group 15-69 years. Conclusions Compared with 1990, the standardized mortality rate and DALY rate decreased in Hubei in 2015, but the death toll and DALYs were still high, especially those caused by pedestrian walkway injury. The disease burden caused by traffic injury were more serious in males than in females.
