产妇与婴儿血清流行性脑脊髓膜炎抗体水平变化特征研究

A longitudinal study of changes of specific antibody against Neisseria meningitidis serogroup A and C in pregnant women and newborns

  • 摘要: 目的 研究流行性脑脊髓膜炎(流脑)抗体在产妇及其婴儿体内的变化情况。方法 选取在广州市某妇幼保健院进行分娩的产妇及其新生儿作为研究对象,采用血清流行病学方法对产妇及新生儿、3月龄和6月龄的流脑抗体水平进行评价。结果 共纳入239名产妇及分娩的婴儿241名。产妇在分娩前A群、C群流脑抗体几何平均浓度(GMC)分别为8.13 g/ml(95%CI:2.89~22.88)和1.37 g/ml(95%CI:0.62~3.05)。新生儿A群流脑抗体GMC为3.60 g/ml(95%CI:0.95~13.63),在3~6月龄时抗体水平在2.85~3.59 g/ml间波动;C群流脑抗体GMC为0.49 g/ml(95%CI:0.15~1.63),至6月龄时抗体水平持续升高5.49 g/ml(95%CI:2.04~14.75)。新生儿较其产妇A群、C群流脑抗体水平均有所下降。母婴A群、C群流脑抗体浓度均呈正相关。结论 新生儿与6月龄婴儿的流脑抗体水平和抗体保护率处临界水平,应加强婴儿与儿童流脑疫苗的免疫接种。

     

    Abstract: Objective To study the level of specific antibody against Neisseria meningitidis serogroup A and C in pregnant women and their infants. Methods A longitudinal sero-epidemiology study was conducted in pregnant women and their infants to investigate the duration of the specific antibody against N. meningitidis serogroups A and C in pregnant women and infants at age of 0, 3 and 6 months. Results A total of 239 pregnant women and 241 infants were included in the study. The geometric mean concentrations (GMC) of specific antibody against N. meningitides serogroup A and C were 8.13 g/ml (95% CI:2.89-22.88) and 1.37 g/ml (95% CI:0.62-3.05), respectively in pregnant women before delivery. The GMC of specific antibody against serogroup A was 3.60 g/ml (95% CI:0.95-13.63) in the newborns and ranged from 2.85 g/ml to 3.59 g/ml in the infants aged 3-6 months, and the GMC of specific antibody against serogroup C increased from 0.49 g/ml (95% CI:0.15-1.63) at age 0 month to 5.49 g/ml (95% CI:2.04-14.75) at age 6 months in the infants. The specific antibody levels to N. meningitides serogroup A and C were lower in the infants than in the pregnant women. A significant positive correlation was observed in specific antibody levels to N. meningitides serogroup A and C between the pregnant women and the infants. Conclusion The specific antibody level to N. meningitides in the infants aged 0 month and 6 months was at critical level. It is necessary to strengthen the vaccination against meningococcal meningitis in infants and children.
