艰难梭菌(Clostridium difficile,CD)是一种革兰阳性的专性厌氧产芽孢杆菌,是引起抗生素相关伪膜性肠炎的病因。当广谱抗生素、化疗药物和免疫抑制剂等药物大量使用破坏了正常肠道菌群平衡时,CD易在肠道定植并导致CD感染(Clostridium difficile infection,CDI)。美国感染控制和流行病学 专业协会(APIC) 的统计数据表明,美国每天CDI患者人数达到7178人,死亡人数为300人[1]。2004年,B I/NAP 1/RT 027型毒力株在加拿大首次出现,接着在欧美国家造成了多次流行与暴发[2]。使CDI的监测成为必须进行的工作。
分子分型是CDI监测中十分重要的方法,主要有表型分型和基因分型两种方法。20世纪80年代分子分型方法仅限于表型分型,包括血清学分型、放射自显影聚丙烯酰胺凝胶电泳(PAGE)和免疫印迹法等。表型分型方法的局限性主要表现在重复性低、分型少和鉴别能力低等方面,无法在流行病学研究中广泛应用,逐渐被基因分型技术所取代。现在常用的CD分子分型方法包括限制性内切酶分析分型(restriction endonuclease analysis typing,REA)、聚合酶链反应核糖体分型(polymerase chain reaction ribotyping,PCR RT)、脉冲场凝胶电泳(pulsed-field gel electrophoresis,PFGE)、多位点序列分析(multilocus sequence typing,MLST)、重复序列PCR分型(repetitive extragenic palindromic-polymerase chain reaction,rep-PCR)、多位点可变数目串联重复序列分析(multilocus variable-number tandem repeat analysis,MLVA)、毒素分型和全基因组测序分析(whole-genome sequencing,WGS)等。现对上述各种分子分型技术做简要综述。
1 艰难梭菌分子分型方法 1.1 基于条带的分子分型方法 1.1.1 REA分型是用 Hind Ⅲ等常见的限制性内切酶对CD完整基因组进行消化,得到的酶切片段用琼脂糖凝胶电泳进行分析,最早在1986年被Kuijper等[3]应用于院内感染CDI传播的研究;Clabots等[4]对1965株CD菌株进行REA分型,共得到75组、206种REA型;Snydman等[5]在对322株CD菌株进行流行病学研究中使用了REA分型,其中BI 菌株占比25.5%。
虽然REA分型稳定性好,但是有报道指出,RT 176、198和244型菌株采用REA分型时,都被错误地判断为RT 027型[6]。这是因为REA分型得到的图谱分辨率低、结果判读困难、重复性差,而且实验室之间的数据平行比对困难,从而限制了REA分型方法的广泛应用。
1.1.2 PCRRT分型 利用16S和23S rDNA基因间隔区多态性进行分型。通过PCR扩增出不同大小的扩增子(一般为200~700 bp),经普通凝胶电泳分离扩增片段,得到的带型即为PCR RT型。目前已报道的两种PCR RT分型方法中[7-8],Stubbs 等[8]的方法具有更高的D值(不同分子分型方法鉴别能力的度量标准为D值,D值在0~1之间,越趋于1说明该分型方法的鉴别能力越高)[9]。PCR RT分型现已能对400多种PCR核糖体型进行区分和甄别。Stubbs 等[8]对2030株CD进行PCR RT分型,得到116种RT型;Walk等[10]对331株CD进行PCR RT分型,得到20种RT型。
传统的PCR RT分型基于琼脂糖凝胶电泳,缺点是缺少足够的分辨率、结果重复性差,有报道指出PCR RT分型无法分辨RT 027、106和017型。因此目前实际工作中基于毛细管电泳的PCR RT分型技术(CE RT分型)逐渐替代了传统的琼脂糖凝胶PCR RT分型,得到越来越广泛的认可。该方法分型识别率高、重复性好、操作相对简便。Fawley等[11]用70株已知RT型的CD菌株建立了一个CE RT分型数据库。
但是由于CD的16S和23S rDNA中重复序列的重组导致新的PCR RT型的产生(概率与诱因皆不清楚),无法与原始菌株保持系统发生学关联。因此,PCR核糖体分型无法追踪CDI暴发,研究CD在患者、动物、食物及医院之间的传播,需要MLVA等分型方法进行补充。
另外,Westblade等[12]报道了一种半自动化的CD分型新方法,该方法将核糖体分型与DiversiLab system、2100 Bioanalyzer相结合,通过半自动化分子分型系统对核糖体分型条带进行分离与分析,D值达到0.954,与PFGE相当。优势在于该方法能够将电子化的条带信息组建成库,并可随时进行回顾性分析。该方法对设备要求很高,尚未广泛应用。
1.1.3 PFGE是用酶切位点稀有的限制性内切酶(如SmaⅠ,SacⅡ)切割CD基因组DNA,得到的大片段DNA采用PFGE进行分离的分型方法,可以用来分离10 kb至10 Mb的DNA分子[13]。最终得到的电泳图谱称为NAP(North American Pulsed-field type,NAP)型。Indra等[14]对146株CD进行PFGE分型,共得到44种NAP型;Aguayo等[15]用PFGE对719株CD得到60种NAP型,其中NAP1占大多数(79%),表明NAP1在智利已广泛传播。
PFGE的缺点是人工投入大且耗时太长,完成一次PFGE实验与结果分析通常需要几天的时间,而且需要专门设备与质控菌株;DNA带型难以解释,特别是当一种带型和标准带型差别很小时;同时,实验结果的说明及实验室之间的数据比对困难。相信通过规定标准化方法、分子质量标准、分析软件并建立公共数据库[类似美国疾病预防控制中心(CDC)的PulseNet计划],PFGE会更加完善并得到广泛的应用。
1.1.4 rep-PCR分型通过扩增细菌基因组中广泛分布的非编码重复序列,经电泳得到指纹图谱显示基因组间的差异。DiversiLab System是基于rep-PCR技术的商品化微生物分型系统,可在4 h之内完成1~13个样本的样本准备、DNA指纹资料捕获及结果分析[16],重复性好,与基于琼脂糖分析的RT分型相比也有着更高的分辨力[17]。Corbellini等[18]采用DiversiLab System对意大利布雷西亚地区采集的59株CD进行了rep-PCR分型,得到20种不同型别。
在针对一家医院的CDI暴发研究中,普通rep-PCR分型有着高分辨力,但是缺少国际通用方法,这使得实验的比较以及实验室间的数据交流变得困难。且DiversiLab System在实验室之间的重复性还需要进一步评估。
1.1.5 MLVA通过对CD基因组中分散各处的短串联重复序列的扩增,用毛细管电泳对扩增片段进行分离与后续自动分析。已报道的其中两种方法[19-20],分别包含7个基因座,其中4个完全相同。每个基因座被指定一个与基因座中重复序列数量相对应的数值,根据计算出的串联重复序列差别总数(summed tandem repeat difference,STRD)建立最小生成树,可以用来衡量遗传差别。STRD≤2时,为相同菌株;STRD≤10时,为遗传相关菌株。
Manzoor等[21]在MLVA基础上建立了eMLVA方法,能够判断出临床意义显著的RT型菌株的亚型,同时与PCR RT分型的分簇,保持一致,该试验中D值高达0.999(PCR RT分型为0.886),但是这一方法的缺点在于需要进行15次PCR,增加了人力负担与成本。
MLVA得到的结果为电子数据,实验周期短;同时有着更高的分辨力,使得追踪CDI暴发以及CD在患者和医院之间的传播时更为高效,同时能够建立菌株之间的系统发生关系。但是MLVA方法仍需要改进,Bakker等[22]指出PCR RT 078型CD的MLVA基因座 A6是一个无效等位基因,因此PCR RT 078 的MLVA分型方法需要进行优化。且MLVA至今未标准化,无法进行实验室间比对。
1.1.6 毒素分型毒素A和B基因 (tcdA,tcdB) 位于CD致病决定区,毒素分型通过PCR扩增tcdA,tcdB基因的10个区段,接着用限制性内切酶消化获得不同大小的片段,根据琼脂糖凝胶电泳得到的特异性图谱进行菌株的分子分型。Rupnik等[23]建立了CD毒素分型标准化方法,该方法有很好的重复性。虽然毒素分型的鉴别力不如PFGE与PCR RT,但是毒素分型针对CD毒素基因(除了二元毒素)进行分型,能够很好地从基因角度阐明不同菌株之间毒素表达的差异。不产毒素的CD菌株(A+B-,A-B+,A-B-)可能因为tcdA或tcdB的缺失而无法用此方法进行分型,而近年来出现了大量A-B+菌株,限制了毒素分型的广泛应用。
1.2 基于测序的分子分型方法 1.2.1 MLST通过对CD的7个管家基因序列片段进行PCR扩增得到长度为300~500 bp的片段,经测序,带有等位基因突变的序列被指定特定的基因序号,7个管家基因的基因序号对应该菌株的序列型(ST型)。目前被广泛认可的MLST方法由Griffiths等[24]于2010年建立,实验结果可以在MLST公众数据库(http://pubmlst.org/cdifficile/)中进行比对,这个开放的数据库也极大地促进了MLST技术在CD分子流行病学研究的开展。近年来,泰国、日本等亚洲国家都有应用MLST技术进行分型的报道[25-26]。
MLST分型的优点在于结果清晰,重复性好,最终数据易于处理与交流共享。MLST数据库中现已有347种ST型(截至2016年3月)。但MLST实验周期较长,数据需自行比对,对于一些基层实验室来说应用仍存在一定困难。
1.2.2 WGS是对未知基因组序列的物种进行个体的基因组测序,拥有现有分子分型中最高的分辨力。通过后续的单核苷酸多态性(single nucleotide polymorphism,SNPs)分析,能够揭示出CD菌株之间小到单个碱基之间的区别。同时,WGS数据能够应用于CD的临床微生物学与流行病学研究。
早在2006年有报道应用Sanger测序方法对CD进行了WGS。2010年,He等[27]用Sanger测序结合二代测序(Roche 454和Illumina)对CD基因组进行测序,阐述了29株CD菌株的系统发生学。Didelot等[28]采用Illumina技术对486株CD进行了WGS分析,估计CD基因组的进化速度约为1.4个突变/基因组/年,并且证实有症状的CDI患者之间的院内传输对于CD感染率的贡献并没有那么大,建议对CD传播途径的研究可转移到健康带菌者等方向上。He等[27]采用WGS进行CD的系统发育分析,发现了两个特别的RT 027/NAP1家系FQR1和FQR2,这两个家系都获得了氟喹诺酮抗性突变基因以及一个密切相关的转座子,此外FQR2传播更为广泛,导致了欧洲、英国和澳大利亚医疗保健机构中的CDI暴发。
全基因组测序的缺点在于数据翻译复杂、费用昂贵,相信在测序成本降低的未来应用将会十分普遍。全基因组测序技术的应用难点在于费用高、耗时长、分析难,现在测序成本持续降低,全基因组测序技术将会越来越普遍,可加深人们对于CD传播途径的理解。
1.3 其他分型方法有一些分子分型方法在CD分型中并不常用,包括质粒指纹图谱分析、随机引物PCR[29]、扩增片段长度多态性分析(amplified fragment length polymorphism,AFLP)[30]、S蛋白A序列分型(slpA ST)[31]、fliC基因RFLP分析[32]等。除此之外,Stojanov等[33]报道了一种双位点序列分型(double locus sequence typing,DLST)方法,D值高于MLST与MLVA,尚未广泛应用。
2 常用分子分型方法应用与比较基于琼脂糖凝胶的PCR RT分型技术在欧洲最为普遍,英国成立了CD核糖体分型网络实验室,主要用于本地、国家以及整个欧洲层面的CD流行病学研究。欧洲学者已建立数据库(the Cardiff collection of Jon Brazier and Val Hall)对新的PCR RT型进行验证。目前,欧洲各国已收集到20种PCR RT型标准菌株,供参与欧洲CDI研究网络实验室计划(ECDISnet)的各参比实验室使用。2011年的广泛统计数据表明[34],引起欧洲院内感染CDI排名前4位菌株分别为RT 014/020(16%),RT 001(10%),RT 078(8%),RT 027型菌株以5%的占比排名第六。
MLST是研究CD系统发生学的好方法。MLST的分辨力与PCR RT分型相当,同时MLST不易受复合事件的影响。因为管家基因的重组只能改变等位基因谱上的一个单基因位点,即使造成ST型的改变,这个新的ST型菌株仍然与原始ST型菌株在系统发生学上保持密切相关。然而在PCR RT分型中,16S和23S rDNA中重复序列的重组导致新的PCR RT型的产生(概率与诱因皆不清楚),却不能与原始菌株保持系统发生学关联。采用MLST系统发育重建研究显示,CD已经进化成为至少5种系谱[35]。
与欧洲普遍应用PCR核糖体分型方法不同,北美多采用PFGE进行分子分型,但是CD的PFGE分型标准化及验证并没有其他一些已建立PulseNet的食源性病原菌那样顺利。有报道指出PFGE比PCR核糖体分型具有更高的分辨力,两者D值分别为0.843和0.688[36],但也有人对39种PCR RT型的CD菌株进行PFGE分型,仅得到16种NAP型[37]。
当应用PCR核糖体分型或MLST对院内感染CDI进行监测时,某一个PCR RT型或ST型数量的增加为CDI的暴发提供了证据。但是这两种方法都无法显示暴发CDI菌株的详细传播途径。MLVA分型弥补了这一不足。荷兰、法国和英国就曾应用MLVA技术调查PCR RT 027型的暴发,揭示出CDI病例之间的潜在联系[38-40]。
现有的较为重要的CD分子分型方法比较实验,在少至两种多至七种方法之间进行了比较。Killgore等[36]比较了REA、PFGE、PCR Ribotyping、MLST、MLVA、AFLP和slpA基因测序分型7种分型方法对42株CD进行分型时的分辨力、分型力以及分组的一致性。7种分型方法D值从高到低(0.964~0.631)排序分别为MLVA、REA、PFGE、slpA、PCR-ribotyping、MLST、AFLP。所有的分型方法都能鉴别BI/027/NAP1型毒力株,但是只有REA和MLVA能够分辨出不同CDI暴发中的菌株。Manzo等[41]的课题组联合全球多家研究中心用PCR RT、REA、RFLP对100株CD进行分型比较,3种方法的D值分别为0.9541、0.9957和0.7353。Dominguez等[42]用rep-PCR、PFGE、WGS对27株CD进行分型比较,二代测序技术分辨出了rep-PCR无法分辨的3个种群,同时认定在PFGE中归类为NAP3型的一株CD菌与其他NAP3型菌株有着3816 SNPs的差别。
WGS与传统分子分型方法相比,虽然成本相对很高,但是鉴于测序成本的逐渐降低以及数据处理速度的不断提高,被认为是前景最为广阔的应用技术,在将来能够涵盖MLST、MLVA等分型方法。标准化的计算流程将作为WGS的数据质控方法,使WGS成为最有可能的通用分型技术。
表 1总结并比较了常用CD分子分型方法采用技术、实验时间所需成本及优缺点,不同的分子分型方法有着各自的优缺点。虽然统一分型方法十分重要,但是大到各个国家、地区,小到各个实验室,研究的重点与实验条件都不相同,应用的分子分型方法也不相同,所以至今没有一个全球通用的CD分子分型方法,研究数据的交流仅限于部分地区或网络实验室之间。

国内对于CD的研究处于起步阶段,并未开展大规模监测,已应用的分子分型技术主要为MLST。Yan等[43]用MLST对104株CD进行系统发生与流行病学研究,得到22种ST型,其中ST37为主要分型,ST117、118、119、129为新的ST型,系统发生学研究中并未发现地理位置或人群相关性,也没有发现地理位置或人群与基因分型之间的关联。Chen等[44]用MLST对161株CD进行分型,共得到30种ST型,其中ST54(23%)、ST35(19.3%)、ST37(10%)占比最多,并未发现ST1 (ribotype 027)或ST11 (ribotype 078)型CD菌株。Wang等[45]用MLST对ICU中CDI感染病例粪便样本中分离得到的28株CD进行分型,最终得到17种ST型,其中11种为新ST型。
4 艰难梭菌分子分型的意义及展望分子分型方法对于CD的流行病学研究有着十分重要的意义。首先,它是临床医生诊断CDI与暴发的重要手段。CDI的即时诊断对于临床医生的意义在于尽早对症下药,并采取措施防止这一感染性病原体在医疗设施中院内传播。其次,分子分型方法能够指导临床用药。非达霉素已用于发达国家CDI的治疗,有报道指出,非达霉素在治疗不同型别的CD时有着不同的功效及复发可能,Louie等[46]的研究就指出,非NAP1型CDI用非达霉素治疗后复发率显著降低。最后,分子分型方法能够用来判断CD地区性与全球性的传播,衡量CD特殊型别的危险性,追溯其长期转变。2005年,PCR RT 027型高毒力株出现,荷兰国家公共与卫生环境研究所设立了CD的国家级参比实验室,监测CD的流行与变异。2009年,该参比实验室发现了一种新的毒力株PCR RT 078型,而这一毒力株随后在欧洲其他国家暴发流行。最近,该参比实验室发现从2010年开始,PCR RT 027型高毒力株又有卷土重来之势。此外,有研究表明RT 176、198、244等新的PCR RT型均由RT 027型进化而来,这意味着这些新的RT型菌株很有可能成为下一个引起暴发流行的菌株。
一个好的分子分型方法得到的结果应该能够将相关菌株进行归类,同时区别无关菌株,具备良好的分型能力(即清晰地判断分型)、重复性(实验室内或实验室间)和较高的成本效益,结果的解释分析也应尽可能客观[47]。现有的分子分型方法各有优缺点。随着测序成本的不断降低,全基因组测序及后续的SNPs分析成为目前最受关注的分子分型方法,有研究者断言全基因组测序将会成为CD分子分型的金标准。液相芯片等成熟的技术也都已应用到CD检测与分型中,CD分子分型方法在分辨力、可重复性以及操作简便程度上都会有更长足的进步。相信在不久的将来,CD研究者们能够更迅速地对CDI暴发进行识别与调查,从而更有效地制定CDI防控政策措施并长期监控变化趋势,尽可能地降低CDI的感染率与致死率。
