浙江省一起输入性登革热暴发的流行病学调查分析

An Epidemiologic Investigation And Analysis of an Imported Dengue Fever Epidemic

  • 摘要: 目的分析浙江省2004年一起输入性登革热暴发的流行病学特征,为制订登革热预防控制措施提供科学依据。方法确定病例定义进行回顾性病例搜索和流行病学调查。采用间接免疫荧光法对疑似病例血清进行登革热抗体检测,用C6/36细胞对急性期病人血清进行病毒分离,以荧光定量RT-PCR方法鉴定。用布雷图指数(BI)法进行蚊蚴密度调查。结果本次疫情流行历时85d,共发病83例,登革热抗体阳性68例,并分离到登革Ⅰ型病毒8株。病例主要集中在某镇的城镇所在地。男性29例,女性54例,发病年龄最小7岁,最大76岁,以20~60岁占78.31%。疫情发生时疫区布雷图指数高达326。经采取以快速杀灭成蚊和清除伊蚊孳生地为主的综合防治措施后,疫情很快得到控制,发现疫情10d后未再出现新发病例。结论本次疫情是由登革Ⅰ型病毒引起。媒介伊蚊大量繁殖,传染源输入是引起此次登革热流行的主要因素。早期病例被误诊,传染源未被及时发现是导致疾病传播的重要原因。迅速灭蚊和消除伊蚊孳生地是控制疫情的有效措施。有必要加强输入病例监测和蚊媒控制工作。

     

    Abstract: Objective this assay was desired tO explore the epidemic characteristics of the outbreak of dengue fever in Zhejiang province in 2004 and to provide a scientific basis for the prevention and control measures.Methods A retrospective epidemiological investigation study was conducted to search the patients by given definition.IFA were used for detecting the and-dengue virus antibodies,and the serum samples were collected from the patients in acute stage for dengue virus isolation with C6/36 cell and the positive samples were identified by RT-PCR.The mosquito larvae density at epidemic spot was measured with BI.Results The period of epidemic caused by type I dengue virus was 85 day.Patients were localized in one town.Gender distribution was male of 29 and female of 54.And age distribution was localized in 20 to 60 years old with the minimum of 7 and the maximum of 76.The BI in epidemic district reached 326 during the epidemic period.Conclusion The outbreak Was caused by dengue virus type I.The first case was imported from Thailand.Diagnostic errors,delaying insulation Was the major reason leading to the outbreak.Eliminating mosquito is the effective method to control and prevent dengue fever.
