舟山群岛孕妇弓形虫感染率及其与不良妊娠结局关系的调查

Investigation report on the relationship between the infection rate of Toxoplasma gondii and adverse pregnancy outcomes among pregnant women in Zhoushan Islands

  • 摘要: 目的 了解舟山群岛孕妇弓形虫感染情况及其与不良妊娠结局的关系.方法 采用酶联免疫吸附法(ELISA)检测5686例孕妇外周血中弓形虫IgG和IgM抗体,追踪观察孕妇的出生结局;对44例畸形胎儿和对照组(188名正常新生儿)检测脐血中IgM.结果 舟山地区孕妇感染弓形虫149人,平均阳性率2.62%,其中既往感染121例(81.21%),新近感染28例(18.19%).感染组孕妇发生自然流产14例(9.39%)、死胎死产4例(2.68%)及胎儿畸形12例(8.05%).未感染组孕妇发生自然流产47例(0.85%)、死胎死产42例(0.76%)及胎儿畸形32例(0.58%).两组比较,差异有统计学意义(P<0.01).感染组的相对危险度(RR)分别为:自然流产12.11、死胎死产3.61、胎儿畸形15.07.先天畸形儿IgM阳性率为27.27%,对照组为0,两组比较,差异有统计学意义(P<0.01).结论 孕妇感染弓形虫可以导致不良妊娠结局,但只要加强对育龄妇女孕前监测及健康教育,由孕妇感染弓形虫感染导致的不良妊娠结局是可以控制的.

     

    Abstract: Objective The present study was conducted to find out the relationship between the infection rate of Toxoplasma gondii and adverse pregnancy outcomes among pregnant women in Zhoushan Islands. Methods Enzyme linked immunoadsorbent assay (ELISA) was adopted to detect toxoplasma gondii antibody-IgG and IgM (ToxIgG/M) in the peripheral blood of 5686 pregnant women, and they were followed up to observe their pregnancy outcomes. Umbilical cord blood samplings were conducted to examine toxoplasma gondii antibody-IgM (ToxIgM) in 44 cases of malformed newborns and 188 normal newborns in the control group. Results In Zhoushan Region, 149 pregnant women were infected by Toxoplasma gondii, including 121(81.21%) cases with past infection and 28 (18.19%) new infection cases,and the average positive rate of infection was 2.62%. The incidences of spontaneous abortion, stillbirths and malformed newborns in the infection group were respectively 14 cases (9.39%) , 4 cases (2.68%) and 12 cases (8.05%), while they were respectively 47 cases (0.85%), 42 cases (0.76%) and 32 cases (0.58%) in the control group. And the difference between the two groups was statistically significant (P0.01). Relative risks (RR) in the infection group were: 12.11 of spontaneous abortion, 3.61 of stillbirths and 15.07 of malformed newborns. The positive rate of ToxIgM among the congenital malformed fetuses was 27.27%, while 0 in the control group. And this difference between the two groups was of statistical significance (P0.01). Conclusion Toxoplasma gondii infection in pregnant women can result in adverse pregnancy outcomes. However, as long as the pre-pregnant monitoring and health education of the women of childbearing age are strengthened, the adverse pregnancy outcomes of pregnant women infected by Toxoplasma gondii can be controlled.
