2007年浙江省居民出生、死亡、传染病、慢性病漏报调查结果分析

Analysis of under-reporting of birth, death, infectious diseases and chronic diseases in Zhejiang province, 2007

  • 摘要: 目的 对浙江省2007年居民漏报调查资料进行分析,得出出生、死亡、传染病和慢性病的漏报率。方法 在全省30个公共卫生监测区内,采用多阶段整群随机抽样的方法抽取93 599户常住居民,根据捕捉-标记-再捕捉的原理,调查过去1年传染病和3年出生及主要慢性病的漏报情况。 结果 本次漏报调查共调查30个县(区)居民 93 599 户, 280 164人。其中城市监测区调查100 658人,占总调查人数的35.93%;农村监测区调查179 505人,占总调查人数的64.07%,性别比1.03∶1,与监测系统人口性别比一致。 查出出生人数5179人,漏报460人,出生漏报率为8.88%,其中城市漏报率19.04%,农村漏报率4.31%。查出死亡人数3447人,漏报183人,居民死亡漏报率合计为5.31%,城市8.18%,农村4.43%。共查出传染病病例403例,漏报37例,漏报率为9.18%,漏诊率为9.44%,城市漏报率6.67%,漏诊率0.94%,农村漏报率10.07%,漏诊率12.09%。查出慢性病病例3077例,漏报1003例,4类主要慢性病的总漏报率为32.60%,其中恶性肿瘤为23.95%,糖尿病为45.97%,冠心病为39.01%,脑卒中为29.69%。结论 经统计学检验,浙江省出生及死亡的漏报率城市均高于农村;传染病漏报率在城市和农村间无差异;4种主要慢性病的漏报率仍然较高,漏报主要以省级和地市级大医院为主。

     

    Abstract: Objective The study was conducted to provide an assessment of under-reporting of birth, death, infectious diseases and chronic diseases in Zhejiang based on the surveillance data in 2007. Methods Based on the Capture-Signed-Capture principle, a multi-stage randomized cluster sample of 93 599 families were selected from 30 public health surveillance areas to analyze the under-reporting of infectious diseases of the past year and birth and major chronic diseases in the past three years. Results A total of 93 599 families from 30 counties (districts) consisting of 280 164 residents were surveyed in the study, including 100 658 urban residents (35.93%) and 179 505 rural residents( 64.07%). The sex ratio was 1.03∶1 which was consistent with the ratio of the general population. The number of birth was 5179, 460 were under-reported (8.88%), the under-reporting rate of birth was 19.04% in urban areas and 4.31% in rural areas. The number of death was 3447, 183 were under-reported (5.31%); the under-reporting rate of death was 8.18% in urban areas and 4.43% in rural areas. A total of 403 cases of infectious disease were found, 37 were under-reported (9.18%), and the under-diagnosing rate was 9.44%. In urban areas, the under-reporting rate of infectious diseases was 6.67% and under-diagnosing rate was 0.94%, in rural areas, the two rates were 10.07% and 12.09% respectively. A total of 3077 cases of chronic disease were found, 1003 cases were under-reported. The overall under-reporting rate of four major chronic diseases was 32.60% (23.95% for malignant tumor, 45.97% for diabetes, 39.01% for coronary artery diseases and 29.69% for apoplexy). Conclusion Statistical results revealed that the under-reporting rates of birth and death were higher in urban areas and in rural areas in Zhejiang, whereas the under-reporting rates of infectious diseases were of no statistical difference. Higher under-reporting rates of four chronic diseases were mostly seen in the large-scale hospitals at provincial and municipal levels.
