2007年北京市昌平区细菌性痢疾菌群分布和耐药谱变化分析

Analysis of the serotype distribution and antibiotic resistance of Shigella in Changping district of Beijing in 2007 

  • 摘要: 目的 了解2007年北京市昌平区细菌性痢疾(菌痢)的病原体菌群分布及耐药特点,为制定预防控制措施、指导临床合理用药提供科学依据。方法 用SS和麦康凯琼脂培养基分离病原体筛选志贺菌,用生化试验和血清学方法鉴定菌群和血清型分型。药敏试验采用临床实验室标准化协会(CLSI,以前称NCCLS)推荐的Kirby-Bauer 法。结果 2007年昌平区志贺菌菌型分布以D 群宋内志贺菌为主,占62.75%;其次为B群福氏志贺菌,占35.29%,B群福氏志贺菌以F4亚型和F1亚型血清型为主;X亚型4株,占22.22%;C群鲍氏痢疾杆菌1株,占1.96%。药敏试验结果:菌株对环丙沙星、氧氟沙星、左氧氟沙星等7药物敏感;对氨苄西林、萘啶酸、庆大霉素、四环素、复方新诺明5种药物耐药。结论 D 群宋内志贺菌取代了既往的B群福氏志贺菌,成为2007年昌平区菌痢的主要流行菌群;B群福氏志贺菌F4亚型和F1亚型取代了既往的福氏2a亚型,成为新的流行血清型;昌平地区的志贺菌耐药谱与北京其他地区既有相似性,也有地区特点,临床上应根据本地区药敏结果合理选择药物。

     

    Abstract: Objective The study was conducted to investigate the serotype distribution and antibiotic resistance of Shigella in Changping district, Beijing in 2007, providing evidence for the development of preventive measures and guidance for proper medication against bacillary dysentery. Methods Shigella samples were cultured and isolated in SS agar and MC media. Serotyping and species indication was performed through biochemical and serological tests. Kirby-Bauer test, recommended by the Clinical and Laboratory Standards Institute(CLSI, the former NCCLS), was used as the susceptibility test. Results The predominant Shigella strain in Changping district in 2007 was Shigella sonnei serogroup D, accouting for 62.75%; following was Shigella Flexneri serogroup B, accounting for 35.29%, consisting mostly of F4 and F1 subtypes. Four isolates of serotype X were identified, amounting to 22.22%, and one isolate was Shigella boydii serogroup C, accounting for 1.96%. Findings of the susceptibility test showed that those strains were sensitive to such seven antibiotics as ciprofloxacin, ofloxacin and levofloxacin, and were resistant to ampicillin, nalidixic acid, gentamicin, tetracycline and compound sulfamethoxazole. Conclusion Having superseding Shigella flexneri serogroup B, Shigella sonnei serogroup D became the predominantly prevailing strain in Changping district in 2007. Meanwhile, subtypes F4 and F1 had superseded subtype 2a as the new prevailing serotypes in Shigella flexneri serogroup B. Both similarities and particularities can be found in the antibiotic resistance spectrum of Shigella when comparing the strains in Changping district with those in other regions of Beijing. Therefore, proper application of antibiotics would be essential in the treatment according to the results of local susceptibility tests.
