浙江省桐乡市蔬菜加工区重点人群碘营养监测结果分析

Analysis of the results of the surveillance on iodine nutrition among key population in vegetable-processing areas in Tongxiang, Zhejiang

  • 摘要: 目的了解浙江省桐乡市蔬菜加工地区重点人群碘营养状况, 评估碘缺乏的流行动态。方法按《全国碘缺乏病监测方案(试行)》规定的方法进行。直接滴定法测定居民户食用盐2307份;以过硫酸铵消解法检测重点人群孕妇、育龄妇女、0~2岁婴幼儿、学龄儿童尿碘582份;采用时间分辨荧光免疫法测定新生儿足跟血促甲状腺激素(TSH)22 246份。结果全市碘盐覆盖率、合格碘盐食用率以2004年最低,分别为89.24%、85.76%。孕妇、0~2岁婴幼儿平均尿碘中位数分别为68.79 g/L、94.02 g/L,部分重点人群尿碘水平低于国家标准,但呈上升趋势。TSH值大于5 mIU/L的比率为5.79%(1290/22 246),呈逐年下降趋势。结论应继续实行全民食盐加碘(USI)补碘措施,抓好合格碘盐覆盖率和重点人群碘营养监测是今后蔬菜加工区的工作重点。

     

    Abstract: ObjectiveTo evaluate the iodine nutrition status among key population from vegetable-processing areas in Tongxiang, Zhejiang, providing evidence for determination of the epidemic trends in regard of iodine deficiency. MethodsA total of 2,307 edible salt samples were measured for iodine contents using direct titration assay in accordance with the national surveillance program on iodine deficiency. Ammonium persulfate digestion was conducted to analyze 582 samples for urinary iodine among pregnant women, women at child-bearing age, 0 to 2-year-old infants and children at school age, respectively. Thyroid stimulating hormone (TSH) levels were measured in heel blood samples for 22 246 neonatal subjects using time resolution fluorescence immunization assay (TRFIA). ResultsThe minimal iodized salt coverage (89.24%) and qualified iodized salt usage (85.76%) were noticed in 2004. The median urinary iodine was 68.79 g/L in pregnant women and 94.02 g/L in 0 to 2-year-old infants, respectively. As a result, the urinary iodine level in major vegetable-processing areas was below national standard despite gradual increment, and 5.79% subjects (1290/22 246) demonstrated the TSH value greater than 5 mIU/L, with a year-by-year decline. ConclusionImportance shall be attached to the improvement in qualified iodized salt coverage and surveillance on iodine nutritional status among key population in vegetable-processing areas in the ensuing universal salt iodization (USI) and iodine supplement program.
