1例阴沟肠杆菌碳青霉烯类抗生素耐药的机制研究

Carbopenems resistant mechanism of a strain of Enterobacter cloacae

  • 摘要: 目的 对浙江省瑞安市人民医院临床分离的1株碳青霉烯类抗生素耐药的阴沟肠杆菌的耐药机制进行研究。 方法 通过E-test检测阴沟肠杆菌及接合菌株的最低抑菌浓度,Hodge试验检测碳青霉烯酶,采用接合实验、聚合酶链反应(PCR)、 质粒抽提、琼脂糖凝胶电泳及耐药基因克隆测序分析细菌的耐药机制。 结果 接合试验显示阴沟肠杆菌中的碳青霉烯类耐药基因位于质粒上,基因扩增、测序、比对等方法测得此耐药基因为BlaKPC-2型。 结论 阴沟肠杆菌碳青霉烯类药物耐药由质粒介导的KPC-2酶引起。

     

    Abstract: Objective To understand the carbopenems resistant mechanism of a strain of Enterobacter cloacae isolated in our hospital. Methods The minimal inhibitory concentrations of the Enterobacter cloacae strain and conjugate bacterium were detected with E-test,the carbapenemase was detected with Hodge test. Conjugation test, polymerase chain reaction (PCR), plasmid extraction, agarose gel electrophoresis and cloning and sequencing of drug resistant gene were conducted to analyze the resistant mechanism. Results Conjugation test indicated that the carbopenems resistant gene of the Enterobacter cloacae strain was in a conjugative plasmid. The gene was identified as BlaKPC-2 through gene amplification, sequencing and comparison. Conclusion The strain of carbapenem resistant Enterobacter cloacae produced KPC-2 carbapenemase.
