2007-2011年浙江省龙游县钩端螺旋体病监测分析

Surveillance of leptospirosis in Longyou, Zhejiang,2007-2011

  • 摘要: 目的 了解浙江省龙游县钩端螺旋体(钩体)病疫情流行特征,为制定防治对策提供参考依据。 方法 用描述性流行病学方法对龙游县2007-2011年钩体病流行特征进行分析,用显微镜凝集实验,对临床确诊病例和宿主动物血清进行抗体测定;采集鼠、蛙、鸭、猪肾和牛中段尿,进行病原体分离培养;采集鼠、蛙肾进行钩体聚合酶链反应(PCR)监测。 结果 龙游县2007-2011年钩体病年平均发病率为0.40/10万,患者以中年农民人群为主,女性高于男性,病例血清抗体对应菌群以黄疸出血群为主;农忙夏收季节是钩体病的高发期,全县多个乡(镇)有发病,以庙下乡发病最多;病原学监测检出钩体菌12株,以秋季热群为主,占83.33%;宿主动物血清抗体阳性率为41%,以黄疸出血群及秋季热群为主,分别占51.22%和39.02%;宿主动物PCR监测阳性率为28.07%。 结论 农忙夏收季节为钩体病高发季节,中年农民为重点人群;宿主动物携带钩体菌群与所检出的患者血清抗体菌群不完全一致,应继续加强对宿主动物和人群的监测,注意菌群变动。

     

    Abstract: Objective To understand the epidemiological characteristics of leptospirosis in Longyou county from 2007 to 2011 and provide scientific evidence for the development of prevention and treatment strategies. Methods Descriptive epidemiological analysis was conducted on the incidence data of leptospirosis in Longyou during this period and the serum samples collected from clinic diagnosed patients and host animals in Longyou were tested by microscopic agglutination test. The kidney samples of mouse, frog, duck, swine and the urine samples of bovine were collected for pathogen culture. The kidney samples of mouse and frog were collected to detect leptospira by PCR. Results The average annual incidence of leptospirosis was 0.40/lakh. Most patients were middle-aged farmers, and more female patients were reported than male ones. The serum antibodies detected in the patients were mainly to L. icterohemorrhagie. The disease mainly occurred in summer and autumn. The cases were distributed in all townships, but the case number in Miaoxia township was highest. Twelve strains of leptospira were detected, most of which belonged to L. autumnalis (83.33%).The positive rate of serum of host animals was 41%, and the main strains were L. icterohemorrhagie (51.22%) and L. autumnalis (39.02%). The positive rate of the kidney samples of mouse and frog was 28.07% by PCR. Conclusion The incidence of leptospirosis was high in summer and autumn in Longyou, and middle-aged farmers were affected mostly. The serogroups of leptospira detected in host animals and patients were not consistent. It is necessary to further strengthen the surveillance of leptospirosis both in human and in host animals.
