普通人群接种病毒性乙型肝炎疫苗免疫效果及安全性观察

Observation of immunological effect and safety of hepatitis B vaccine in general population

  • 摘要: 目的 观察普通人群接种病毒性乙型肝炎(乙肝)疫苗后免疫效果及安全性。方法 选择16~49岁成年人3178人,接种10 g乙肝疫苗3剂,采集免疫前后2份血清,第1次采血与接种第1针疫苗同步进行,第2次采血在全程接种后1个月。运用化学发光法定量检测乙肝病毒表面抗原(HBsAg)、抗乙肝病毒表面抗体(抗-HBs)和抗乙肝病毒核心抗原抗体(抗-HBc)。结果 免疫前抗-HBs阴性者接种疫苗后阳转率97.68%,不同年龄组阳转率差异有统计学意义(2=17.28,P=0.001);免疫前抗-HBs阴性者接种后抗-HBs几何平均浓度(GMC)为 1123.31 mIU/ml;免疫前抗-HBs阳性者接种疫苗前后GMC分别为 63.32 mIU/ml和 9415.59 mIU/ml,比较前后GMC差异有统计学意义(t=71.141,P=0.00)。结论 普通人群接种乙肝疫苗后,近期能取得良好的免疫效果,其长期效果有待进一步观察。

     

    Abstract: Objective To evaluate the immunological effect and safety of hepatitis B vaccine in general population. Methods Totally 3178 adult aged 16 to 49 years were selected to administer 3 doses of hepatitis B vaccine (10 g). The first blood samples were taken while the first hepatitis B vaccine was given, the second blood samples were taken 1 month after the completion of the series vaccination. All the blood samples were detected for hepatitis B virus (HBV) surface antigen (HbsAg), antibody to hepatitis B virus surface antigen (anti-HBs) and antibody to hepatitis B virus core antigen (anti-HBc) by using chemiluminescence assay. Results For the people who were anti-HBs negative before vaccination, the positive conversion rate was 97.68%, the age specific differences were statistical significant (2=17.28,P=0.001). The geometric mean concentration (GMC) of anti-HBs was 1123.31 mIU/ml. For the people who were anti-HBs positive, the GMC of anti-HBs were 63.32 mIU/ml and 9415.59 mIU/ml before and after vaccination, the difference was statistical significant (t=71.141, P=0.00). Conclusion The short term effect of hepatitis B vaccine immunization is good in general population, but the long term effect needs further observation.
