Value of tuberculous infection of T cells spot test in adult pulmonary tuberculosis diagnosis

摘要: 目的探讨γ干扰素释放分析结核感染T细胞斑点试验(Tuberculous infection of T cells spot test,T-SPOT.TB)在成年人肺结核诊断中的意义。方法对2011年3－12月在南京医科大学附属杭州市第一人民医院住院治疗的74例成年人疑似肺结核患者进行外周血T-SPOT.TB 检测,其结果与最终诊断相比较,并与结核菌素试验(芒图试验, PPD)进行对比。结果 T-SPOT.TB灵敏度为93.5%, 特异度为92.1%。在确诊结核病组中,T-SPOT.TB 阳性率显著高于PPD试验(45.2%),差异有统计学意义(P结论 T-SPOT.TB检测对成年人肺结核诊断有高度的敏感性和特异性,值得临床推广。

Abstract: Objective To evaluate the value of an interferon-gamma release assay-tuberculous infection of T cells spot test (T-SPOT.TB) in adult pulmonary tuberculosis (TB) diagnosis. Methods T-SPOT.TB was conducted in 74 suspected pulmonary TB cases by using their peripheral blood in our hospital from March to December 2011. The T-SPOT.TB results were compared with the final diagnoses as well as the results of PPD test. Results The sensitivity and specificity of T-SPOT.TB test were 93.5% and 92.1% respectively. The positive rate of T-SPOT.TB test was significantly higher than that of PPD test (45.2%) in adult pulmonary TB diagnosis (PConclusion T-SPOT.TB has higher sensitivity and specificity than other traditional methods such as PPD test and could be widely used in hospitals.
