临床分离志贺菌中产CTX-M型超广谱β-内酰胺酶基因型及耐药性分析

Genotype and drug resistance of CTX-M extended spectrum beta-lactamases in clinical isolates of Shigella

  • 摘要: 目的 了解北京地区临床分离志贺菌携带CTX-M型超广谱β-内酰胺酶(ESBLs)耐药基因型及耐药特征。 方法 收集2008-2011年本地区分离出的志贺菌83株,聚合酶链反应(PCR)检测CTX-M型ESBLs耐药基因,明确基因型,阳性产物序列在GenBank上进行比对确定基因亚型,并通过DNAman软件对核酸序列进行分析;对携带CTX-M型ESBLs耐药基因菌株按照Kirby-Bauer法进行药物敏感性试验,测定志贺菌对8种头孢类抗生素耐药情况。 结果 83株志贺菌中,13株携带CTX-M型ESBLs基因,阳性率为15.66%。对DNA序列进行比对、分析均为CTX-M-9型中的CTX-M-14亚型,未发现单核苷酸多态性(SNPs)位点。药敏结果显示,13株携带CTX-M型ESBLs基因菌株对头孢噻吩、头孢唑林、头孢噻肟等头孢类抗生素耐药,对头孢西丁、头孢他啶、头孢吡肟和头孢哌酮等头孢类抗生素均敏感,部分菌株对氨曲南耐药。 结论 本地区志贺菌中CTX-M型ESBLs以CTX-M-14亚型为主,基因结构稳定,携带CTX-M-14型ESBLs菌株呈多重耐药。

     

    Abstract: Objective To understand the genotype and drug resistance of CTX-M extended spectrum beta-lactamases(ESBLs) in clinical isolates of Shigella in Xicheng district of Beijing. Methods Totally 83 strains of clinical isolates of Shigella were colleted in Xicheng district from 2008 to 2011. CTX-M ESBLs was detected by polymerase chain reaction(PCR), then the sequencing for PCR positive products was performed, the sequencing results were aligned with those in GenBank to determine gene subtypes, meanwhile the nucleotide sequence was analyzed by using DNAman software. The drug susceptibility test of the strains carrying CTX-M ESBLs gene to 8 kinds of cephalosporins was performed by Kirby-Bauer method. Results Among 83 strains of Shigella, 13(15.66%) carried CTX-M ESBLs gene. The results of DNA sequence analysis showed that CTX-M ESBLs were all CTX-M-14 subtype, and no single nucleotide polymorphism(SNPs) site was found. The results of drug susceptibility test showed that 13 strains carrying CTX-M ESBLs were resistant to cephalothin, cefazolin and cefotaxime. However, they were sensitive to cefoxitin, ceftazidime, cefepime and cefoperazone, but some strains were resistant to aztreonam. Conclusion The CTX-M ESBLs in Shigella isolated in Xicheng district of Beijing were mainly CTX-M-14 subtype and the gene structure was stable. The strains carrying CTX-M-14 ESBLs showed multidrug resistance.
