浙江省杭州市结核分枝杆菌氧氟沙星耐药研究

Drug resistance of Mycobacterium tuberculosis to ofloxacin in Hangzhou

  • 摘要: 目的 了解杭州市结核分枝杆菌对氧氟沙星的耐药情况及耐药菌株gyrA基因的突变特点。 方法 收集2010年4月至2012年6月杭州市各结核病定点医院分离培养的临床菌株,进行一线抗结核药物、氧氟沙星和卡那霉素敏感性检测,测定氧氟沙星耐药菌株gyrA基因的喹诺酮类药物耐药决定区(QRDR)序列。 结果 收集的1242株临床菌株中, 67株(5.39%)对氧氟沙星耐药,其中32株(47.76%)对氧氟沙星单耐药,23株为耐多药结核病(MDR-TB)菌株,占MDR-TB菌株的22.77%。选取55株耐药株进行gyrA序列分析,其中33株(60%)携带gyrA基因突变位点。突变位点包括第90和94位密码子,突变率分别为27.27%和32.73%。 结论 杭州市氧氟沙星单耐药患者的比例较高,MDR-TB患者中氧氟沙星耐药情况较严重,需加强结核患者的早期发现和用药管理。

     

    Abstract: Objective To understand the resistance of Mycobacterium tuberculosis to ofloxacin in Hangzhou and the characteristics of gyrA mutation in ofloxacin resistant M. tuberculosis. Methods The clinical isolates of M. tuberculosis were collected from every TB hospitals in Hangzhou from April 2010 to June 2012 to test their susceptibility to first-line drug, ofloxacin and kanamycin. The sequence of the quinolone resistance-determining region (QRDR) in gyrA gene of the ofloxacin resistant isolates was detected. Results A total of 1242 M. tuberculosis isolates were detected, in which 67 (5.39%) were resistant to ofloxacin. Among the 67 strains, 32 (47.76%) were resistant only to ofloxacin strains, 23 were multi-drug-resistant, accounting for 22.77% (23/101) of total multi-drug-resistant strains. Fifty five genomic DNA samples from the ofloxacin resistant strains were selected for gyrA gene sequence analysis. Results showed that gyrA mutation was found in 33 isolates (60%), in which 27.27% were at codon 90 and 32.73% were at codon 94. Conclusion The resistance to ofloxacin was high both in mono-drug-resistant TB and multi-drug-resistant TB patients in Hangzhou. It is necessary to strengthen the early detection of TB patients and the management of clinical antibiotic use.
