引起老年患者院内下呼吸道感染的肺炎克雷伯菌中质粒介导的AmpC酶基因型检测与分析

Genotype of plasmid-mediated AmpC -lactamases in Klebsiella pneumoniae causing noscomial lower respiratory tract infection in elder patients

  • 摘要: 目的 探讨引起老年患者院内下呼吸道感染的肺炎克雷伯菌中质粒介导的AmpC酶基因型,为产AmpC酶细菌的监测和防治提供依据. 方法 采用头孢西丁纸片对产AmpC酶菌株进行初筛;双纸片表型确证法确证产AmpC酶;采用UNIQ-10柱式质粒小量抽提试剂盒提取细菌质粒;采用聚合酶链反应(PCR)和DNA测序检测分析质粒AmpC酶基因型. 结果 双纸片表型确证试验检出17株产AmpC酶菌株,检出率为17.5%.经PCR扩增17株菌株均在405 bp处出现阳性条带,经DNA测序证实为DHA-1型质粒AmpC酶. 结论 浙江省临海市中医院存在质粒介导AmpC酶流行菌株,质粒AmpC酶主要以DHA-1型为主.

     

    Abstract: Objective To understand the AmpC -lactamases genotype of the plasmids mediated in Klebsiella pneumoniae causing nosocomial lower respiratory tract infection among elder patients and provide evidence for the surveillance of the bacteria producing AmpC -lactamases and the prevention and control of related infection. Methods Cefoxitin disk diffusion test was conducted to detect the phenotype of AmpC -lactamases, and AmpC -lactamases was confirmed by dual paper phenotypic method. The plasmids was extracted from bacterium with minipreps kit UNIQ-10. Polymerase chain reaction (PCR) and DNA sequencing were conducted to detect the genotype of AmpC -lactamase of the plasmids, Results A total of 17 K. pneumoniae strains producing AmpC -lactamase genotype DHA-1 were detected, accounting for 17.5%. Conclusion Klebsiella pneumoniae producing plasmid-mediated AmpC -lactamase circulated in Linhai County Traditional Chinese Medicine Hospital, and the plasmid-mediated AmpC -lactamase mainly belonged to genotype DHA-1.
