2010-2016年湖北省人间布鲁氏菌病流行病学特征分析

Epidemiological characteristics of human brucellosis in Hubei, 2010-2016

  • 摘要: 目的 了解湖北省2010-2016年人间布鲁氏菌病(布病)流行特征,为布病防治提供科学依据。方法 利用传染病报告信息管理系统中2010-2016年湖北省人间布病个案数据,分析布病疫情流行特征、病例就诊等情况。利用突发公共卫生事件报告管理信息系统中湖北省布病暴发疫情数据,分析暴发疫情特点。结果 2010-2016年湖北省共报告布病947例,年均报告发病率为0.23/10万,发病呈现逐年增加趋势(趋势2=838.312,P=0.001)。病例以农民为主(71.28%)。发病县(区)覆盖率为74.76%(77/103),波及县(区)从2010年2个增至2016年53个,呈逐年增加趋势(趋势2=138.278,P=0.001)。发病至诊断时间间隔中位数为13 d;省级医院报告病例比例呈逐年降低趋势(趋势2=66.027,P0.05),县级医院报告病例比例呈逐年增加趋势(趋势2=31.427,P=0.001)。2010-2016年共报告2起暴发疫情,可疑的暴露因素均为接触染疫的羊只。结论 2010-2016年布病在湖北省呈现由北至南逐年扩增的趋势;基层医疗机构对布病诊断水平不断提高。建议加强布病联防联控,遏制布病的增长势头。

     

    Abstract: Objective To understand the epidemiological characteristics of human brucellosis in Hubei province from 2010 to 2016 and provide scientific evidence for human brucellosis prevention and control. Methods The incidence data and outbreak information of human brucellosis in Hubei during this period were collected from Communicable Disease Reporting Information System and Public Health Emergency Reporting Information System respectively for a descriptive analysis. Results A total of 947 cases of human brucellosis were reported in Hubei during 2010-2016,with an average incidence of 0.23/100 000 and an increasing trend year by year(trend 2=838.312,P=0.001). The male to female ratio of the cases was 2.47:1,and the median age of cases was 50 years. Most cases were farmers(71.28%). The incidence peak was from April to July(49.52%). The counties where human brucellosis occurred accounted for 74.76%(77/103),the affected counties increased from 2 in 2010 to 53 in 2016(trend 2=138.278,P=0.001). The incidence was highest in Suizhou(1.95/100 000),followed by Jingmen (0.54/100 000)and Xiangyang(0.38/100 000). The median of the interval between onset and diagnosis was 13 days,and the proportion of the cases reported by hospitals at provincial level decreased with year(trend 2=66.027,P0.05)and that reported by county hospitals increased with year(trend 2=31.427,P=0.001). Two outbreaks of human brucellosis were reported during 2010-2016,sick sheep were suspected infection sources. Conclusion The incidence of human brucellosis increased year by year with the affected areas expanding from the north to the south in Hubei during 2010-2016. The diagnosis capability of human brucellosis had been improved gradually at medical facilities at grass roots. It is suggested to conduct joint prevention and control of human brucellosis to stop the increase of human brucellosis incidence in Hubei.
