2011-2016年安徽省安庆市发热伴血小板减少综合征监测分析

Surveillance for severe fever with thrombocytopenia syndrome in Anqing,Anhui,2011-2016

  • 摘要: 目的 分析安徽省安庆市发热伴血小板减少综合征(SFTS)疫情的流行特征,为制定其防控措施提供科学依据。方法 采用描述性流行病学方法对SFTS疫情监测及流行病学调查资料进行分析。结果 2011-2016年安庆市共报告SFTS 316例,年均报告发病率为1.02/10万,发病率呈上升趋势(趋势χ2=12.533,P=0.001),死亡11例,病死率为3.48%,病死率呈下降趋势(趋势χ2=2.798,P=0.005);病例主要分布在安庆市北部大别山地区的桐城市、潜山县和岳西县(66.77%),每年4月发病开始上升,5-10月为高发季节,男女性发病率、病死率差异无统计学意义(χ2=0.225,P=0.635;2χ=0.154,P=0.695),发病年龄集中在45~75岁年龄组(84.81%),死亡病例年龄均在50岁以上,职业以农民为主(83.86%);病例居住地为山区或丘陵者占88.32%,2周内有户外劳作史者占64.96%,有蜱叮咬史者占13.14%;病例标本检测阳性率为38.57%。结论 安庆市SFTS发病持续上升,时空范围扩大,感染者以中老年农民为主,桐城市、潜山县和岳西县是安庆市的主要自然疫源地;应加强疫源地区农村中老年人群的防控工作,提高其野外作业时的防护意识。

     

    Abstract: Objective To analyze the epidemiological characteristics of severe fever with thrombocytopenia syndrome (SFTS)in Anqing,Anhui province,and provide scientific evidence for the prevention and control of the disease. Methods Descriptive epidemiological analysis was conducted by using the surveillance and epidemiological survey data of SFTS in Anqing during 2011-2016. Results A total of 316 SFTS cases were reported in Anqing during this period,the average annual incidence was 1.02/100 000,its incidence was in increase(P<0.001). Eleven cases were fatal,the case fatality rate was 3.48%,showing a decrease trend(P=0.005). The cases were mainly distributed in Tongcheng,Qianshan and Yuexi (66.77%)in the north of Anqing. The annual incidence began to increase in April and the peak season was during MayOctober. The differences in incidence and case fatality rate were not significant between males and females (χ2=0.225, P=0.635;χ2=0.154,P=0.695). The cases mainly occurred in age group 45-75 years(84.81%),all the death cases were aged >50 years. Most cases were farmers(83.86%). Up to 88.32% of the cases lived in mountain or hill area. The cases with field work history and with tick bite history within 2 weeks accounted for 64.96% and 13.14% respectively. The positive rate in case sample detection was 38.57%. Conclusion The incidence of STFS was in increase in Anqing,and its temporal and spatial distribution expanded. Middle aged and elderly farmers were mainly affected. Natural foci were mainly distributed in Tongcheng,Qianshan and Yuexi. It is necessary to strengthen prevention and control of SFTS and health education in the elderly in these areas.
