1950-2018年浙江省杭州市甲乙类传染病疾病谱分析

Changes of class A and B communicable disease spectrum in Hangzhou, 1950–2018

    目的分析1950 — 2018年浙江省杭州市甲乙类传染病发病特点和流行趋势,明确传染病防控重点。
    方法从历年传染病报告工作报表和“中国传染病报告信息管理系统”中收集1950 — 2018年杭州市甲乙类传染病疫情信息,以10年为1个阶段,应用趋势χ2检验比较各阶段甲乙类传染病发病率、死亡率和病死率趋势,比较分析不同时期甲乙类传染病疾病谱和发病顺位。
    结果1950 — 2018年期间,7个阶段甲乙类传染病平均发病率、平均死亡率和平均病死率总体呈现明显下降趋势(χ2=2 608 369.10、32 135.20和5 953.34,P<0.001)。 2010 — 2018年平均发病率、死亡率、病死率与1950 — 1959年相比分别下降89.68%、98.20%和82.20%。 呼吸道传染病、肠道传染病、虫媒和自然疫源性疾病发病率均大幅度下降,7个阶段甲乙类传染病发病顺位变化较大,淋病、梅毒、肺结核取代早期的麻疹、疟疾、百日咳,进入发病前5顺位。 艾滋病是在后3个阶段才出现的新传染病,是增长幅度最大且持续增长的严重传染病。
    结论1950 — 2018年杭州市甲乙类传染病发病率、死亡率明显下降,传染病疾病谱发生明显变化,建议高度关注性传播疾病(特别是艾滋病)、肺结核、人感染H7N9禽流感、登革热等传染病防治。

     

    Abstract:
    ObjectiveTo understand the incidence characteristics and epidemic trends of class A and B communicable diseases in Hangzhou from 1950 to 2018, clarify the focus of the prevention and control of epidemics.
    MethodsThe epidemic data of class A and B communicable diseases in Hangzhou from 1950 to 2018 were collected from the annual communicable disease reporting forms and "National Notifiable Disease Information Management System". A 10-year period division was used for the related analysis in different period from 1950 to 2018. The incidence trends and case-fatality of class A and B communicable diseases were assessed with χ2 test. The spectrum and rank of class A and B communicable diseases in different periods were analyzed.
    ResultsFrom 1950 to 2018, the average morbidity rate, average mortality rate and average case-fatality rate of communicable diseases in Class A and B showed a steady decline trend ( χ2trend=2 608 369.10, 32 135.20 and 5 953.34, P<0.001). Compared with 1950–1959 period, the average morbidity rate, mortality rate, and case-fatality rate in 2010–2018 period declined by 89.68%, 98.21%, and 82.20% respectively. The incidences of respiratory infections, intestinal infectious diseases, insect vector-borne diseases, and natural focus infectious diseases in Hangzhou decreased significantly. The incidence rank of class A and B communicable diseases changed greatly in 7 periods. Gonorrhea, syphilis, and tuberculosis have become the diseases in top five diseases replacing measles, malaria, and whooping cough in early periods. HIV/AIDS was only began to report in the latest three periods, but the incidence showed an obvious and consistent increase.
    ConclusionThe morbidity rates and mortality rates of class A and B communicable diseases decreased obviously in Hangzhou from 1950 to 2018. The disease spectrum has changed greatly. In the future, close attention should be paid to the prevention and control of sexually transmitted diseases, especially HIV/AIDS, tuberculosis, human infection with avian influenza A (H7N9) virus, and dengue fever etc.
