浙江省湖州地区10680例女性人乳头瘤病毒感染及基因分型分析

Analysis on human papilloma virus infection among 10 680 females in Huzhou, Zhejiang

  • 摘要: 目的 了解浙江省湖州地区女性人乳头瘤病毒(human papilloma virus,HPV)亚型分布状况及年龄相关性,为本地区女性宫颈癌的防治及HPV疫苗的研制提供科学参考。方法 收集2011年1月至2013年12月于湖州市中心医院就诊的10 680份女性宫颈脱落细胞标本,以基因芯片方法进行HPV-DNA亚型检测及统计学分析。结果 10 680份标本,检出HPV阳性标本2865例,感染率为26.83%(2865/10 680)。亚型分布构成比排在前3位的分别为HPV16为17.52%(718/4099),HPV52为15.32%(628/4099),HPV58为12.42%(509/4099)。高危感染占82.41%(2361/2865),低危感染占8.90%(255/2865),高低危混合感染8.69%(249/2865);20岁年龄组3种感染类型感染率均最高。感染亚型以单一亚型感染为主(69.56%),单一亚型感染率最高的是45~49岁年龄组为77.56%,多重亚型感染率最高的是20岁年龄组为60.38%。结论 湖州地区门诊女性HPV感染率为26.83%,亚型以HPV16,18,52为主。20岁以下高感染率、高多重感染率说明HPV感染有低龄化多亚型化倾向,值得引起高度重视。

     

    Abstract: Objective To understand the hypo-type distribution of human papilloma virus (HPV) in infected women and its relationship with age in Huzhou, Zhejiang province, and provide evidence for the cervical cancer prevention and control in local women and the development of HPV vaccine. Methods HPV-DNA detection was conducted by using cervical exfoliated cell samples collected from 10 680 females seeking medical care in our hospital from January 2011 to December 2013. Results Among the 10 680 samples, 2865 were detected to be HPV positive (26.83%). The HPV hypo-type detection indicated that the first 3 types were HPV16 (17.52%), HPV52 (15.32%) and HPV58 (12.42%). The high risk infection accounted for 82.41% (2361/2865), and the low risk infection accounted for 8.90% (255/2865). The high/low risk co-infection accounted for 8.69% (249/2865). The infection rates of low risk type, high risk type and co-infection rate of high/low risk type were highest in age group20 years. The simplex type infection accounted for 69.56%. The simplex type infection rate was highest in age group 45-49 years (77.56%), the multi type infection rate was highest in age group20 years (60.38%). Conclusion The HPV infection rate was 26.83% among the females seeking medical care in our hospital. HPV16, 18 and 52 were predominant. The high infection rate and high multi type infection rate in age group 20 years suggested the younger age trend and multi type trend of HPV infection, to which much attention should be paid.
