浙江省德清县1例人感染H7N9禽流感病例流行病学调查与分析

Epidemiological investigation and analysis of one case of human infection with avian influenza A (H7N9) virus in Deqing, Zhejiang

  • 摘要: 目的 探索2016年浙江省德清县1例人感染H7N9禽流感病例的感染来源,为防控人禽流感提供科学依据。方法 调查确诊病例和密切接触者,采集外环境标本进行实验室检测。结果 患者确诊为人感染H7N9禽流感病例,经救治无效死亡。患者年龄76岁,有高血压病史,发病前7天无活禽接触史,有农贸市场暴露史,虽然该农贸市场无活禽交易,但市场内杀白禽摊位处检出1份H7N9流感病毒核酸阳性的污水标本。3名杀白禽摊主的活禽关养宰杀场所均有H7N9流感病毒核酸阳性标本检出。13名密切接触者均未出现急性呼吸道感染症状。结论 病例感染来源于受H7N9病毒污染的农贸市场的可能性较大,建议加强农贸市场的卫生管理和消毒工作,规范禽类宰杀与销售。

     

    Abstract: Objective Toidentify the possible infection source of one case of human infection with avian influenza A (H7N9) virus reported in Deqing, Zhejiang province, in January 2016, and provide scientific evidence for the prevention and control of avian influenza infection in human. Methods The case and his close contacts were surveyed, and external environment samples were collected for laboratory testing. Results The patient,aged 76 years, was confirmed to be infected with human avian influenza A (H7N9) virus, and eventually died despite the treatment was given. The case had history of hypertension. The case had exposure history to a farm produce market but no history of exposure to alive poultry within 7 days before onset. Although there were no alive poultry in the farm produce market, one polluted water sample was detected to be positive for H7N9 viral nucleic acid at a killed poultry stall. Positive samples of H7N9 viral nucleic acid were detected in poultry cages and slaughter sites in all of the three killed poultry stalls. No acute respiratory symptoms were found in the 13 close contacts of the patient. Conclusion It was highly possible that the case was infected in the farm produce market contaminated by H7N9 virus, indicating that health management and disinfection in farm produce market need to be strengthened, and poultry slaughtering and sale procedure need to be standardized.
