2017-2022年云南省艾滋病高危人群人类免疫缺陷病毒新近感染监测结果分析

Surveillance for human immunodeficiency virus infection in population at high risk for acquired immune deficiency syndrome in Yunnan, 2017−2022

      目的  了解云南省女性商业性服务者和性病门诊男性就诊者人类免疫缺陷病毒(HIV)新近感染情况,为艾滋病防治策略制定和评估提供参考。
      方法  采用连续横断面研究方法在艾滋病监测哨点开展女性商业性服务者和性病门诊男性就诊者中HIV新近感染监测,使用限制性抗原亲和力酶联免疫法(LAg法)对HIV抗体阳性样本进行新近感染检测,根据通用公式计算HIV新近感染率。
      结果  2017—2022年云南省女性商业性服务者哨点共开展HIV检测64 058人份,HIV阳性率为0.36%~0.89%,呈下降趋势(趋势χ2=27.024,P<0.001);HIV新近感染率为0.05%~0.27%,无直线线性变化趋势(r=0.090,P=0.865)。 性病门诊男性就诊者哨点共开展HIV检测31 642人份,HIV阳性率为0.53%~1.00%,无直线线性变化趋势(趋势χ2=3.076,P=0.079);HIV新近感染率为0.20%~0.31%,无直线线性变化趋势(r=−0.558,P=0.208)。
      结论  云南省女性商业性服务者和性病门诊男性就诊者HIV感染率和新近感染率维持在相对较低水平,持续开展新近感染监测对艾滋病疫情研判具有重要意义。

     

    Abstract:
      Objective  To understand the infection status of human immunodeficiency virus (HIV) in female sex workers (FSWs) and male patients with sexually transmitted diseases (STDs) and influencing factors in Yunnan province during 2017−2022.
      Methods  A cross-sectional study was used in HIV sentinel surveillance in FSWs and male STD patients, and HIV-positive samples were tested for recent infections by LAg–Avidity enzyme immunoassay. The recent HIV infection rate was calculated according to the recommended general formula.
      Results  A total 64 058 HIV tests were conducted in FSWs in sentinel surveillance in Yunnan during this period, and the HIV positive rate ranged from 0.36% to 0.89%, showing an overall downward trend (trend χ2=27.024, P<0.001), and the recent HIV infection rate ranged from 0.05% to 0.27%, showing no obvious linear change trend (r=0.090, P=0.865). A total of 31 642 HIV tests were conducted in male STD patients in sentinel STD clinics in Yunnan during this period, and the HIV positive rate ranged from 0.53% to 1.00%, showing no linear change trend (trend χ2=3.076, P=0.079). And the recent HIV infection rate ranged from 0.20% to 0.31%, showing no linear change trend (r=−0.558, P=0.208).
      Conclusion  The infection rate and recent infection rate of HIV remained low in FSWs and male STD patients in Yunnan. Continuous surveillance for recent infection of HIV is important for the assessment of acquired Immune deficiency syndrome epidemic in Yunnan.
