2023年浙江省台州市一起人间布鲁氏菌病暴发疫情调查分析

Epidemiological investigation of a human brucellosis outbreak in Taizhou, Zhejiang, 2023

    目的  通过对浙江省台州市某养殖场1起布鲁氏菌病(布病)暴发疫情进行调查分析,为布病防治工作提供依据。
    方法 通过现场流行病学调查和实验室检测,对2023年6月台州市仙居县养殖场发生的1起布病聚集性疫情进行溯源调查和传染途径分析,对调查结果和实验室检测结果进行描述流行病学分析。
    结果  本次暴发疫情共发现4例确诊病例及2例隐性感染者,病例发病时间为2023年4月1日至8月15日。 临床表现主要为反复发热、乏力、肌肉关节痛和睾丸肿大等。 4例病例中3例从事养殖场病羊管理或饲养工作,1例负责病羊无害化处置。 畜间血清学检测结果显示,采集该养殖场羊血样58份,检出阳性55份,阳性率94.83%。 经溯源调查,本次暴发疫情的阳性羊只来源于浙江省桐乡市可能性大。
    结论  本次台州市暴发疫情中发现在无害化处理环节发生布病感染,提示在今后布病防控工作中,需将无害化处置人员列为重点职业人群监测对象,开展布病防治知识健康宣教及危险行为干预,降低布病感染风险。

     

    Abstract:
    Objective To understand the epidemiological characteristics of a human brucellosis outbreak in Taizhou, Zhejiang province, and provide evidence for the prevention and control of brucellosis.
    Methods Field epidemiological survey and laboratory test were conducted for the human brucellosis outbreak in a local livestock farm in June 2023 to identify infection source and transmission route. Descriptive epidemiological methods were used to analyze the investigation results and laboratory detection data.
    Results A total of 4 brucellosis cases and 2 latent infection cases were found in the farm from April 1 to August 15 in 2023. The main clinical manifestations were recurrent fever, fatigue, muscle and joint soreness, and orchitis. In the 4 cases, 3 were engaged in the management or raising of sick goats, and 1 participated in the harmless treatment of sick goats. The serological test results showed that 55 goats in the farm were positive for Brucella (94.83%, 55/58). The investigation indicated that the positive goats might be imported from Tongxiang in Zhejiang.
    Conclusion In this brucellosis outbreak, infection occurred in the harmless treatment process of sick goats. In the future, the workers involved in harmless treatment of sick animals should be included in the occupational surveillance, and health education should be carried out to reduce the risk for brucellosis.
