2015-2025年中国无偿献血者梅毒感染率的Meta分析

Meta-analysis on infection rate of Treponema pallidum in voluntary blood donors in China, 2015−2025

    目的 了解我国无偿献血者梅毒感染率及影响因素,为制定针对性的献血宣传和血源招募策略提供科学依据。
    方法 检索Pubmed、维普、万方、中国知网等数据库中有关中国无偿献血者梅毒感染的文献,检索时限为2015年5月至2025年5月,使用加强流行病学中观察性研究报告声明对纳入文献质量评估,使用 Stata 18.0对无偿献血者梅毒感染率进行Meta分析,按照地区、年龄、调查时间、职业、学历、性别、婚姻状况、献血次数进行亚组分析。
    结果 纳入32篇研究文献,共6518349人次,其中梅毒感染21401人次,梅毒感染率为318.99/10万(95%CI:259.21/10万~378.77/10万),亚组分析显示,不同性别组、不同婚姻状况组梅毒感染率差异无统计学意义(P>0.05),不同地区组、不同年龄组、不同调查时间组、不同职业组、不同学历组、不同献血次数组梅毒感染率差异有统计学意义(P<0.05)。西部地区、东部地区及中部地区无偿献血者梅毒感染率分别为:521.30/10万、375.50/10万及176.97/10万;18~<35岁组、35~<45岁组、45~60岁组无偿献血者梅毒感染率分别为:207.72/10万、419.60/10万、471.03/10万;2004-2015和2016-2022年无偿献血者梅毒感染率分别为432.40/10万和226.90/10万;学生群体和其他群体无偿献血者梅毒感染率分别为107.06/10万和371.25/10万;大学及以上学历组和高中及以下学历组无偿献血者梅毒感染率分别为122.74/10万和393.10/10万; 首次献血者和重复献血者梅毒感染率分别为541.37/10万和57.19/10万。
    结论 我国无偿献血者梅毒感染率在全球处于中等水平,且近年来大幅下降,不同地区间差异较大,无偿献血者梅毒感染率随着年龄的增长而升高,学生、受教育程度较高及重复献血者为梅毒感染的低风险群体,为高血液安全人群。

     

    Abstract:
    Objective To understand the infection rate of Treponema pallidum and influencing factors in voluntary blood donors in China, and provide scientific evidence for the development of targeted blood donation promotion and blood source recruitment strategies.
    Methods Literature retrieval of publications about T. pallidum infection in voluntary blood donors in China from May 2015 to May 2025 was conducted by using databases such as Pubmed, Wanfang, and China National Knowledge Infrastructure. Strengthening the reporting of observational studies in epidemiology statement was used to evaluate the quality of the included literature and meta-analysis on syphilis infection rate in the voluntary blood donors was conducted by using Stata 18.0. Subgroup analysis was performed in terms of area, age, survey time, occupation, education level, gender, marital status and the number of blood donations.
    Results A total of 32 papers were included, involving 6518349 blood donors. Among them, 21401 blood donors were infected with T. pallidum, the infection rate was 318.99/100000 (95%CI: 259.21/100000−378.77/100000). Subgroup analysis showed that no significant differences in T. pallidum infection rates among different gender and marital status groups (P<0.05), while significant difference in T. pallidum infection rates was found among the blood donors in different area, age, survey time, occupation, education level, and blood donation time groups (P<0.05). The T. pallidum infection rates in the blood donors in western, eastern, and central China were 521.30/100000, 375.50/100000 and 176.97/100000, respectively; the T. pallidum infection rates in the blood donors in age groups 18−<35, 35−<45 and 45−60 years were 207.72/100000, 419.60/100000 and 471.03/100000, respectively; the T. pallidum infection rate in the blood donors was 432.40/100000 during 2004−2015 and 226.90/100000 during 2016−2022; the T. pallidum infection rate in the blood donors was 107.06/100000 in student group, and 371.25/100000 in other groups; the T. pallidum infection rates in the blood donors with education level of university or above and high school or below were 122.74/100000 and 393.10/100000, respectively; the T. pallidum infection rates in the first-time and repeated blood donors were 541.37/100000 and 57.19/100000, respectively.
    Conclusion The T. pallidum infection rate in the voluntary blood donors in China was at moderate level compared with other countries, but it decreased significantly in recent years. Students, highly educated individuals and those who has repeated blood donation are low-risk groups for T. pallidum infection, indicating that they can be defined relatively safe blood donors.
