2022-2024年湖北省天门市居民死因监测结果分析

Surveillance for causes of death in residents in Tianmen, Hubei, 2022 - 2024

    目的 通过分析 2022-2024年湖北省天门市常住居民的死因状况和死因顺位,及疾病负担变化趋势,为制定科学的公共卫生干预措施提供依据。
    方法  依据《国际疾病分类第十版(ICD-10)》对死因进行分类编码,应用SPSS 27.0统计软件计算粗死亡率、标准化死亡率、潜在减寿年数(PYLL)、平均减寿年数(AYLL)等指标;采用χ2检验性别、年份间死亡差异是否有统计学意义。
    结果 2022-2024年湖北省天门市的全人群粗死亡率为 678.38 /10 万,标准化死亡率为446.20/10万,粗死亡率呈逐年上升趋势(χ2=110.564,P<0.001);居民前五位死因依次为:恶性肿瘤、心脏病、脑血管疾病、损伤及中毒、呼吸系统疾病,粗死亡率分别为:168.21/10万,161.93/10万,128.83/10万,76.64/10 万,69.63/10 万。前五大死因所造成的死亡数占全市累计报告死亡数的 83.28%。前五位死因的PYLL为41856.34人年, AYLL为 5.50人年,潜在减寿率为 27.14‰,其中损伤和中毒对寿命损失影响最大。
    结论  肺癌、肝癌等恶性肿瘤、损伤与中毒等慢性非传染性疾病,以及急性心肌梗死等缺血性心脏病对天门市居民的生命健康造成极大的威胁。因此,湖北省天门市应对这些疾病采取积极的防控措施,推动早诊早治和危险因素干预,以提高居民期望寿命。

     

    Abstract:
    Objective  To understand the causes of death and the rank of causes of death, as well as the changing trends in disease burden in permanent residents in Tianmen, Hubei province, from 2022 to 2024 and provide evidence for the development of effective public health intervention measures.
    Methods The causes of death were coded according to the International Classification of Diseases, 10th Revision (ICD-10). Satistical software SPSS 27.0 was used to calculate the crude mortality rate, standardized mortality rate, potential years of life lost (PYLL), and average years of life lost (AYLL), and χ2 test was used to evaluate the significance in the differences between men and women and among different years.
    Results The overall crude mortality rate was 678.38/100 000, and the standardized mortality rate was 446.20/100 000 in population in Tianmen from 2022 to 2024. The annual crude mortality rate showed an increasing trend (χ2=110.564, P<0.001). The top five causes of death in the residents were: malignant tumor, heart disease, cerebrovascular disease, injury and poisoning, and respiratory disease, with mortality rates of 168.21/100 000, 161.93/100 000, 128.83/100 000, 76.64/100 000, and 69.63/100 000, respectively. The deaths caused by the top five causes of death accounted for 83.28% of the total reported deaths. The PYLL from the top five causes of death was 41,856.34 person-years, the AYLL was 5.50 person-years, and the PYLL rate was 27.14‰. Injury and poisoning had the greatest impact on the life expectancy of local residents.
    Conclusion Malignant tumors, such as lung cancer and liver cancer, chronic non-communicable diseases, such as injury and poisoning, and ischemic heart diseases, such as acute myocardial infarction, all pose significant threats to the lives and health of residents in Tianmen. Therefore, it is necessary to conduct proactive and targeted prevention and control, promote early diagnosis and treatment and strengthen risk factor intervention to improve life expectancy of residents in Tianmen.
