中老年人丧偶时长与抑郁症状、慢性病数量的关联—基于中国健康与养老追踪抽样调查

Association of widowhood length with depressive symptoms and number of chronic diseases in middle-aged and elderly people in China-based on china health and retirement longitudinal study

    目的 探究丧偶时长与中老年人抑郁症状和慢性病数量的关联以及性别和年龄差异,提出相应的建议,以改善其健康状况。
    方法 利用中国健康与养老追踪调查数据,将2011年随访至2018年的5 642名中老年人信息作为研究资料,采用多水平增长模型分析丧偶时长与中老年人抑郁症状和慢性病数量的关联。
    结果 丧偶与中老年人抑郁症状和慢性病数量有关,丧偶1年内抑郁症状风险增加(β=3.62,95%CI:2.17~5.06),丧偶5年后抑郁症状下降,不同性别结果相似,然而45~<65岁群体比高龄老年人效应更显著(β=4.00,95%CI:1.88~6.12)。丧偶初期与慢性病数量减少有关(β=−3.05,95%CI:−3.52~−2.57),不同年龄、性别群体均有此规律。在丧偶11年后,慢性病数量增加(β=0.62,95%CI:0.24~1.01),该效应在女性和年轻老年人群体中更显著。
    结论 丧偶与中老年人抑郁症状的增加相关,且对抑郁的影响是急性且持久的,不同性别差异一致;丧偶对慢性病数量的影响是短期内呈负向关联,11年后增加患病风险,女性的影响比男性更显著;与高龄老年人相比,丧偶对年轻老年人的身心健康会造成更显著的负面影响。

     

    Abstract:
    Objective To explore the association between widowhood length and the prevalence of depressive symptoms and the number of chronic diseases and its gender and age specific differences, and to propose corresponding suggestion to improve their health status
    Methods The information of 5 642 middle-aged and elderly people in follow up from 2011 to 2018 of China Health and Retirement Longitudinal Survey (CHARLS) were used, and a multilevel growth model was used to analyze the associations between the length of widowhood and the prevalence of depressive symptoms and the number of chronic diseases in the middle-aged and elderly people.
    Results Widowhood was associated with the prevalence of depressive symptoms and the number of chronic diseases in the middle-aged and elderly adults, with a significant increase in the prevalence of depressive symptoms within 1 year of widowhood (β=3.62, 95%CI: 2.17−5.06), and a decrease in the prevalence of depressive symptoms after 5 years of widowhood, with similar gender specific results, however, the effect was more significant in age group 45~64 years than in older age group (β=4.00, 95%CI:1.88−6.12). Widowhood was initially associated with a reduction of number of chronic diseases in both men and women and in different age groups (β=−3.05, 95%CI: −3.52−2.57), after 11 years of widowhood, the number of chronic diseases increased (β=0.62, 95%CI: 0.24−1.01), with stronger effect in women and in younger age group.
    Conclusions Widowhood is associated with increased prevalence of depressive symptoms in middle-aged and elderly adults, showing acute and lasting effects in both men and women. While widowhood shows a short-term negative association with the number of chronic diseases and would increase the risk for chronic diseases 11 years later, with a more significant effect in women. Widowhood has more significant negative effect on the physical and mental health of younger age group compared with older age group.
