2017-2024年内蒙古自治区利福平耐药肺结核患者流行情况分析

Epidemiological characteristics of rifampicin-resistant pulmonary tuberculosis in Inner Mongolia Autonomous region, 2017–2024

    目的  分析2017-2024年内蒙古自治区(内蒙古)利福平耐药肺结核患者流行病学特征,为制定有效防控耐药肺结核防控措施提供依据。
    方法  收集2017-2024年全国传染病报告信息管理系统中登记的内蒙古利福平耐药肺结核患者病案信息,采用率及构成比进行描述性统计分析。
    结果 2017-2024内蒙古肺结核患者71198例,发病率呈逐年下降的趋势,由2017年的40.09/10万,下降到2024年的30.39/10万。其中利福平耐药肺结核患者2123例,各年度报告发病率在1.08/10万左右,占总患者数的2.98%,利福平耐药患者比例成上升趋势(趋势χ2=40.808,P<0.001);男性患者1536例,女性患者587例,男女性别比为2.62∶1;年龄分布主要集中在45~<55岁年龄组;治疗分类以新患者为主,占49.41%,其次为复发患者,占33.35%;耐药肺结核患者中合并糖尿病的比例显著上升(趋势χ2=7.159,P=0.007),由2019年的1.59%上升到2024年的21.29%;
    结论  内蒙古地区肺结核患者登记率呈下降趋势,利福平耐药肺结核患者登记率保持在1.08/10万左右,但利福平耐药肺结核患者比例逐年上升,应加强耐药肺结核防控工作。

     

    Abstract:
    Objective To analyze the epidemiological characteristics of rifampicin-resistant pulmonary tuberculosis (RR-TB) in Inner Mongolia Autonomous Region(Inner Mongolia) from 2017 to 2024, and provide evidence for development of effective prevention and control of drug-resistant pulmonary TB.
    Methods The case information of RR-TB in Inner Mongolia from 2017 to 2024 were collected from National Infectious Disease Reporting Information Management System, and descriptive statistical analysis was conducted by using incidence rate and constituent ratio of the cases.
    Results A total of 71 198 pulmonary TB cases were registered in Inner Mongolia from 2017 to 2024, with the incidence rate showing a decrease from 40.09/100 000 in 2017 to 30.39/100 000 in 2024, in the pulmonary TB cases, 2 123 were RR-TB cases, with an annual reported incidence rate of about 1.08/10 000, accounting for 2.98% of all the pulmonary TB cases. The constituent ratio of RR-TB cases showed an upward trend (trend χ2=40.808, P<0.001). In the RR-TB cases, 1 536 were men and 587 were women, with a male-to-female ratio of 2.62∶1. The RR-TB cases were mainly distributed in age group 45−<55 years, and the RR-TB cases were mainly new cases (49.41%), followed by relapsed cases (33.35%). the proportion of the cases with diabetes in drug-resistant and sensitive pulmonary TB cases increased significantly from 1.59% in 2019 to 21.29% in 2024 (trend χ2=7.159, P=0.007,χ2=17.032, P<0.001).
    Conclusion The registration rate of pulmonary TB cases decreased in Inner Mongolia from 2017 to 2024, while the registration rate of RR-TB cases remained at about 1.08/100 000. However, the proportion of RR-TB cases increased year by year, and efforts should be made to strengthen the prevention and control of drug-resistant pulmonary TB.
