2024年甘肃省28株艰难梭菌基因组分析

Genomic analysis on 28 Clostridioides difficile isolates from Gansu, 2024

    目的 艰难梭菌感染(CDI)是医疗相关腹泻的首要病因,但我国甘肃省重点地区CDI相关研究较为缺乏。本研究旨在通过对该地区腹泻患者的流行菌株进行分子分型与进化分析,揭示其CDI阳性率、菌株分子特征及地域分布规律,为制定针对性防控策略提供关键科学依据。
    方法 收集2024年1-12月甘肃省临夏回族自治州、兰州市、白银市、定西市、金昌市、酒泉市共6个地市的1109份腹泻患者粪便/肛拭子样本,筛选艰难梭菌核酸阳性样本,经厌氧培养分离菌株。对分离株进行全基因组测序,利用PubMLST数据库进行序列分型(ST)。分析耐药基因携带情况及基于核心基因组多位点序列分型(cgMLST)的系统发育关系。
    结果 所收集的1109份样本中,核酸检测总阳性率为9.20%(102/1109),其中临夏回族自治州阳性率最高(16.81%,38/226),其次为兰州市(11.48%,14/122)。厌氧培养共分离获得28株艰难梭菌,分离率为27.45%(28/102)。测序结果显示28株菌中产毒株占82.14%(23/28),其中3株携带二元毒素基因(cdtA+/cdtB+);经ST分型共鉴定出15种ST型,以ST3(6株)、ST5(3株)、ST42(3株)和ST54(3株)为主,其中酒泉市菌株ST型呈多样性。耐药基因分析显示50.00%(14/28)的菌株携带≥3类抗生素耐药基因,提示其多重耐药潜力;cgMLST系统发育分析显示部分甘肃分离株与国内其他省份菌株亲缘关系较近,存在跨省传播的可能。
    结论 甘肃省CDI存在地域差异,临夏回族自治州的高流行率与酒泉市优势基因型的传播风险需予以重点关注。建立的28株基因组数据库为后续全省MLST分型数据库奠定基础。下一步结合药敏试验与临床资料,进一步优化防控策略。

     

    Abstract:
    Objective Clostridioides difficile infection(CDI)is a leading cause of healthcare-associated diarrhea. However, the research on CDI in key areas of Gansu province remains limited. This study aims to investigate the epidemiological and molecular characteristics of C. difficile isolated from diarrhea patients in this province, and provide scientific basis for development of targeted prevention and control strategies.
    Methods From January to December 2024, a total of 1109 stool samples or anal swabs were collected from diarrhea patients in 6 prefectures (municipality) in Gansu: i.e. Linxia Hui Autonomous Prefecture(Linxia), Lanzhou, Baiyin, Dingxi, Jinchang, and Jiuquan. The samples were tested for C. difficile by nucleic acid detection, followed by anaerobic culture for strain isolation. All the isolates underwent whole-genome sequencing. Sequence types (STs) were determined by using PubMLST database. Phylogenetic relationships based on core genome multilocus sequence typing (cgMLST) and the carriages of drug resistance genes were analyzed.
    Results The overall nucleic acid positive rate was 9.20% (102/1109). Linxia had the highest positive rate (16.81%, 38/226), followed by Lanzhou (11.48%, 14/122). Anaerobic culture yielded 28 C. difficile isolates, with an isolation rate of 27.45% (28/102). Sequencing revealed that 82.14% (23/28) of the isolates were toxigenic, with three strains carrying binary toxin genes (cdtA+/cdtB+). A total of 15 STs were identified, predominated by ST3 (n=6), ST5 (n=3), ST42 (n=3), and ST54 (n=3). the strains from Jiuquan showed the highest ST diversity. The drug resistance gene analysis showed that 50.00% (14/28) of the strains carried genes conferring resistance to three or more classes of antibiotics, indicating a multidrug resistance potential. The phylogenetic analysis based on cgMLST revealed that some strains isolated in Gansu had close genetic relationships with strains from other provinces in China, suggesting possible cross-provincial transmission.
    Conclusion The incidence of C. difficile infection varied in Gansu. It is necessary to pay close attention to the high infection incidence in Linxia and the transmission risk associated with the predominant genotypes in Jiuquan. The genomic data obtained from these 28 isolates established a foundation for a provincial MLST database. Future studies should integrate the data from drug susceptibility testing and clinical practice to further improve local prevention and control of C. difficile infection.
