2025年山西省太原市一起由肠炎沙门菌污染市售蛋糕引起的食源性疾病暴发流行病学调查及溯源分析

Epidemiological investigation and tracing of a foodborne disease outbreak caused by Salmonella Enteritidis contaminated cake in Taiyuan, Shanxi, 2025

    目的 针对2025年7月山西省太原市A、B区2起疑似关联食源性疾病事件,调查明确暴发原因、污染来源及传播路径,为跨门店食源性疾病精准防控提供依据。
    方法 运用描述流行病学方法分析病例临床特征、流行病学特征及相关危害因素;通过病例-对照研究确定可疑食物;开展食品卫生学调查查明可疑食品关键污染风险环节;采集环境、食品及生物样本,并进行相关致病菌检测;对分离菌株进行全基因组测序,通过核心基因组多位点序列分型(cgMLST)分析及聚类分析,进行病原菌溯源分析。
    结果 本次事件累计暴露376人,发病49人,整体患病率为13.03%;病例临床表现以腹泻(95.92%)、发热(79.59%)、呕吐(44.90%)和腹痛(42.86%)为主。食用榴莲千层蛋糕是发病的危险因素(OR=35.21,95%CI: 9.37~132.26,P<0.001)。卫生学调查发现,店长使用私家车混装运输原料及半成品,车辆未专车专用且无清洁消毒记录,存在交叉污染风险。从加工间操作台、冰箱、蛋糕半成品、成品涂抹样以及店员、患者粪便样品中均分离到肠炎沙门菌,菌株间核心基因差异≤2个等位基因(匹配率≥92%),属于同一克隆系。临床病例分离株、食品及环境分离株在系统发育树中高度聚类,不同来源菌株间具有高度遗传同源性。
    结论 本次事件是一起因源头潜在污染,叠加运输与加工环节违规操作导致的跨区域食源性疾病暴发。私家车混装运输及门店不规范加工引发交叉污染,致使致病菌扩散至终端产品。连锁蛋糕店应建立严格的供应商审核与索票索证制度;各门店须严格执行生熟分开等基本卫生规范,禁止使用非专用车辆转运食品原料/半成品。

     

    Abstract:
    Objective  To understand the causes, infection sources, and transmission routes of two suspected foodborne disease outbreaks in July 2025 in district A and B in Taiyuan, Shanxi province, and provide evidence for precise foodborne disease prevention and control.
    Methods Descriptive epidemiology method was used to analyze the clinical characteristics, epidemiological characteristics and related risk factors of the cases. Case-control study was conducted to identify the suspicious food. Food hygiene investigation was carried out to find out the key contamination risks of the suspicious food. Environmental, food and biological samples were collected and tested for relevant pathogens. Whole-genome sequencing was performed for the isolated strains, and pathogen tracing analysis was conducted through core genome multilocus sequence typing (cgMLST) and cluster analysis.
    Results A total of 376 people were exposed, with 49 cases were infected, the attack rate was 13.03%. The main clinical manifestations of the cases were diarrhea (95.92%), fever (79.59%), vomiting (44.90%) and abdominal pain (42.86%). Eating durian mille-feuille cake was the risk factor (OR=35.21, 95% CI: 9.37−132.26, P<0.001). Food hygiene investigation found that the cake store manager used private car to transport raw materials and semi-finished products, and the car had no cleaning and disinfection records, indicating a cross-contamination risk. Salmonella Enteritidis was isolated from the processing table, refrigerator, semi-finished products, finished products of the cake store, as well as the stool samples from the store staff and the cases. The core gene difference among the strains was ≤2 alleles (matching rate ≥92%), suggesting that they belonged to the same clonal complex. Clinical case strains, food and environmental isolates were highly clustered in the phylogenetic tree, and strains from different sources shared high genetic homology.
    Conclusion This was a foodborne disease outbreak due to the spread of pathogen to end products caused by potential source contamination in transportation and irregular processing in district A and B. It is suggested for cake chain stores to establish a strict supplier audit and invoice/certificate verification system, strictly implement basic hygiene standards such as separation of raw and cooked foods, and prohibit the use of non-specialized vehicle for the transportation of raw materials/semi-finished products of the foods.
