Surveillance for echinococcosis in Gansu, 2020−2024

    目的 分析2020-2024年甘肃省棘球蚴病疫情监测动态变化,为下一步防控工作提供参考依据。
    方法 收集整理2020-2024年甘肃省棘球蚴病监测信息,采用描述性流行病学的研究方法对棘球蚴病新发病例、居民监测、小学生监测、终末宿主监测、中间宿主监测、小学生棘球蚴病防治知识知晓情况及健康行为养成情况进行统计分析。
    结果 2020-2024年甘肃省共报告棘球蚴病例953例,男女性别比1∶1.08,平均年龄57.50(57.50±15.08)岁,报告地主要为武威市天祝藏族自治县(118例)、庆阳市环县(109例)、白银市靖远县(56例)、张掖市山丹县(45例)、敦煌市(41例),职业以农民为主(75.24%)。共筛查631 854人,共检出棘球蚴患者100例,检出率总体呈现逐年下降趋势(χ2_trend=40.053,P<0.05)。筛查小学生243 033人,检出棘球蚴病患者1例,各年份检出率差异无统计学意义(P=0.763)。不同年份间家犬棘球绦虫粪抗原阳性率差异有统计学意义(χ2=46.459,P<0.05),呈现先降低后升高的趋势(χ2_trend=15.787,P<0.05)。不同年份间无主犬与野外犬科动物棘球绦虫粪抗原阳性率差异有统计学意义(χ2=37.905,P<0.05),基本呈逐年降低趋势(χ2_trend=24.537,P<0.05)。不同年份间牲畜患病率差异有统计学意义(χ2=39.960,P<0.05);啮齿目动物感染率差异无统计学意义(χ2=3.596,P=0.463)。不同年份间小学生棘球蚴病防治知识知晓率差异有统计学意义(χ2=2 558.703,P<0.05),“不与狗接触”行为养成率最低,平均养成率为63.79%,“不用生的家畜病变脏器直接喂狗”行为养成率最高,平均养成率为93.43%。
    结论 2020-2024年甘肃省棘球蚴病病例报告病例数、居民棘球蚴病检出率、无主犬与野外犬科动物粪棘球绦虫抗原阳性率呈下降趋势,家犬棘球绦虫粪抗原阳性率、牲畜患病率、小学生棘球蚴病防治知识知晓率和健康行为养成率仍存在波动,但不同区域间存在显著差异,需持续落实综合防控措施,控制以至阻断棘球蚴病流行和传播。

     

    Abstract:
    Objective  To analyze the dynamic changes of echinococcosis prevalence in surveillance sites in Gansu province from 2020 to 2024 and provide evidence for future prevention and control efforts.
    Methods  Surveillance data of echinococcosis in Gansu from 2020 to 2024 were collected for a descriptive analysis on the epidemiological characteristics of echinococcosis cases, surveillance results in local residents, surveillance results in schoolchildren, definitive host surveillance results, intermediate host surveillance results, as well as the awareness rate of knowledge about echinococcosis prevention and the prevalence of healthy behaviors in schoolchildren.
    Results A total of 953 cases of echinococcosis were reported from 2020 to 2024, the male-to-female ratio of the cases was 1∶1.08 and the average age of the cases was 57.50 years (57.50±15.08). The cases were mainly reported in Tianzhu Tibetan autonomous county in Wuwei (118 cases), Huanxian county in Qingyang (109 cases), Jingyuan county in Baiyin (56 cases), Shandan county in Zhangye (45 cases), and Dunhuang (41 cases). The majority of the cases were farmers (75.24%). A total of 631854 individuals were screened at the surveillance sites in Gansu, and 100 cases were detected. The overall detection rate showed a declining trend (χ2trend= 40.053, P<0.05). In 243,033 schoolchildren screened, 1 case was detected, no significant difference in annual detection rates was found (P=0.763). The annual positive rate of Echinococcus coproantigen in domestic dogs varied significantly (χ2=46.459, P<0.05), showing a decrease then decrease trend (χ2trend=15.787, P<0.05). The differences in annual coproantigen positive rate of Echinococcus in stray and wild canines showed significant difference (χ2=37.905, P<0.05), with an overall declining trend observed over time (χ2trend=24.537, P<0.05). The annual prevalence of echinococcosis in livestock varied significantly (χ2=39.960, P<0.05), while the infection rate in rodents showed no significant difference (χ2=3.596, P=0.463). Significant differences were observed in annual awareness rate of knowledge about echinococcosis prevention in schoolchildren (χ2=2 558.703, P<0.05). In terms of healthy behaviors in schoolchildren, the prevalence rate of “contacting no dogs” was lowest, with an average of 63.79%, while that of “feeding no raw, diseased livestock viscera directly to dogs” was highest, with an average of 93.43%.
    Conclusion From 2020 to 2024, the reported echinococcosis cases, the echinococcosis detection rate in local residents, and the positive rate of Echinococcus coproantigen in stray dogs and wild canines in Gansu showed declining trends. However, the fecal Echinococcus granulosus antigen positive rate in domestic dogs, livestock infection rate, the awareness of the knowledge about echinococcosis prevention and control and the prevalence rate of healthy behaviors in schoolchildren still exhibited fluctuations, with significant area specific difference. It is necessary to continuously implement comprehensive prevention and control measures to control the prevalence and ultimately interrupt the transmission of echinococcosis.
