2003-2025年我国分离的178株炭疽芽孢杆菌体外抗生素敏感性和耐药机制分析

In vitro antibiotic susceptibility and resistance mechanism of 178 Bacillus anthracis strains isolated in China from 2003 to 2025

    目的 调查2003-2025年我国分离出的炭疽芽孢杆菌体外抗生素的敏感性特征。
    方法 采用E-test法测试了178株炭疽芽孢杆菌对13种抗生素的药物敏感性;通过基因组比对分析青霉素和氨苄西林的耐药机制。
    结果 试验结果显示,所有菌株对阿莫西林、环丙沙星、克拉霉素、克林霉素、多西环素、左氧氟沙星、利福平、美罗培南等8种抗生素均敏感;发现1株对青霉素和氨苄西林耐药,1株对氯霉素耐药;97.19%(173/178)的菌株对头孢曲松呈中度敏感。基因组分析提示,青霉素与氨苄西林耐药可能与rsiP基因第154位核苷酸缺失导致的预测蛋白截短有关,rsiP 基因第154位核苷酸缺失为新的耐药相关变异位点。
    结论 我国近年分离的炭疽芽孢杆菌对多数推荐预防及治疗药物敏感;本研究为全面了解炭疽芽孢杆菌耐药现状提供了新的数据,有助于临床合理选用抗感染药物及制定防控策略。

     

    Abstract:
    Objective To investigate the in vitro antibiotic susceptibility of Bacillus anthracisB. anthracis)strains isolated in China from 2003 to 2025.
    Methods  The drug susceptibility of 178 strains of B. anthracis to 13 antibiotics was tested by using E-test method; the resistance mechanisms to penicillin and ampicillin were investigated through comparative genomic analysis.
    Results  The antibiotic susceptibility testing showed that all the tested strains were sensitive to amoxicillin, ciprofloxacin, clarithromycin, clindamycin, doxycycline, levofloxacin, rifampicin, and meropenem. The resistance to penicillin and ampicillin was observed in 1 strain, and 1 strain was resistant to chloramphenicol. The majority of the strains ( 97.19% ) exhibited intermediate sensitivity to ceftriaxone. Genomic analysis suggested that resistance to penicillin and ampicillin might be associated with a nucleotide deletion mutation at position 154 in rsiP gene, representing a novel drug resistance-associated variant.
    Conclusion Most B. anthracis isolated in China in recent years remained sensitive to the recommended antibiotics for postexposure prophylaxis and treatment. This study provides updated data for the better understanding of antibiotic resistance of B. anthracis and reasonable selection of antibiotics for treatment and prevention.
