2025年天津市国家突发急性传染病防控队员能力画像分型与建设策略分析

Capability profiling and construction strategy of national emergency response team for acute infectious disease control and prevention in Tianjin, 2025

    目的  以天津市国家突发急性传染病防控队(以下简称“天津市国家队”)全部在岗队员为对象,描绘其应急能力的整体水平、内部差异和职能分布,为我国应急队伍的分类化建设提供参考。
    方法  2025年11月对146名天津市国家队队员开展应急能力评估问卷调查(全员普查),问卷内容包括调查对象一般信息,以及队伍管理、现场指挥协调、快速反应、风险评估、现场调查、检验检测、现场处置、风险沟通、信息化和后勤保障10项能力的信息。以满分的 80.00%为达标基准,采用单样本t检验联合Cohen's d描述能力结构;对10项能力得分内中心化后进行K-均值聚类(K=3),以识别能力画像;低于满分的80.00%定义为“精准培训重点对象”,运用logistic回归分析其相关影响因素。
    结果 共发放问卷146份,回收有效问卷138份,有效回收率94.52%。天津市国家队应急能力总分为(137.57±23.87)分、得分率为81.40%±14.12%,与80.00%的达标基准比较差异无统计学意义(t=1.168,P=0.245)。10项能力中的6项(队伍管理、现场指挥协调、快速反应、风险评估、现场处置、后勤保障)得分率高于基准,差异均有统计学意义(均P<0.01),构成该队的核心优势能力域;现场调查、风险沟通和信息化3项能力得分率与基准比较,差异均无统计学意义(均P>0.05),属于稳健发展区;检验检测得分率仅为62.00%±20.70%,仅16名队员实际承担实验室检测任务,反映该项能力呈“专业化集约建设”特征,是面向高精尖防控的战略性提升方向。K-均值聚类将138名队员划分为3类画像:全能领先型39人(28.26%)、管理见长型75人(54.34%)和潜力发展型24人(17.39%)。多元logistic回归显示,控制其他变量后,后勤保障组队员被识别为优先精准培训对象的可能性高于专业处置组(校正比值比=2.62,95%置信区间:1.15~5.99,P=0.022)。
    结论  2025年天津市国家队整体应急能力达标,组织管理与应急响应是最稳定的能力支柱,队员之间的差异可被概括为3类画像。建议围绕优势能力的巩固、专业化能力的提升、按画像分层的培训以及对后勤保障组的重点赋能等方向推进队伍建设。

     

    Abstract:
    Objective To describe the overall level, internal differences, and functional distribution of emergency response capacity of all active members of the National Emergency Response Team for Acute Infectious Disease Control and Prevention in Tianjin Municipality (hereinafter referred to as the “Tianjin national team”), and provide evidence for the classified development of emergency response teams in China.
    Methods In November 2025, a questionnaire survey was conducted in 146 members of the Tianjin national team to collect the information about their general demographic characteristics and 10 domains of emergency response capability, including team management, on-site command and coordination, rapid response, risk assessment, field investigation, laboratory testing, on-site response, risk communication, informatization, and logistics support. A score of 80.00% of the full score was used as the benchmark for qualification. One-sample t tests combined with Cohen’s d were used to describe the capability structure. K-means clustering (K=3) was performed after within-person centering of the 10 capability scores to identify capability profiles. Members scoring below 80.00% of the full score were defined as “priority targets for precise training,” and Logistic regression model was used to analyze associated factors.
    Results A total of 146 questionnaires were distributed, and 138 valid questionnaires were returned (94.52%). The overall emergency response capability score of the Tianjin national team was 137.57±23.87, with a scoring rate of 81.40%±14.12%. The difference was not significant based on 80.00% benchmark (t=1.168, P=0.245). Six of the 10 capability domains—team management, on-site command and coordination, rapid response, risk assessment, on-site response, and logistics support—had scoring rates significantly higher than the benchmark, forming the core strength domains of the team (all P<0.01). The scoring rates for field investigation, risk communication, and informatization did not differ significantly from the benchmark (all P>0.05), indicating a stable development. The scoring rate for laboratory testing was only 62.00%±20.70%, and only 16 members were actually engaged in laboratory testing, suggesting a specialized and intensive pattern of capability development in this domain and identifying it as a key area for improvement in high-level infectious disease prevention and control. K-means clustering classified the 138 members into three capability profiles: all-round leading type (39 members, 28.26%), management-oriented type (75 members, 54.34%), and potential development type (24 members, 17.39%). Multivariable Logistic regression analysis showed that, after adjustment for other variables, rhe members of the logistics support group were more likely to be identified as priority targets for precise training compared with those in the professional response group (adjusted odds ratio=2.62, 95% confidence interval: 1.15−5.99, P=0.022).
    Conclusion In 2025, the overall emergency response capability of the Tianjin national team reached the benchmark of qualification. Organizational management and emergency response capability were the most stable pillars of team performance, and inter-individual differences among team members could be classified as three capability profiles. Future team development may focus on consolidating existing strengths, improving specialized capabilities, implementing profile-based tiered training, and strengthening capability building for the logistics support group.
