2005-2024年安徽省六安市百日咳流行病学特征分析

Epidemiological characteristics of ertussis in Lu'an, Anhui, 2005–2024

    目的  分析2005—2024年安徽省六安市百日咳流行病学特征,为优化防控策略及评估免疫程序调整效果提供依据。
    方法  利用中国疾病预防控制信息系统和安徽省免疫规划信息管理系统数据,采用描述性流行病学方法分析六安市百日咳的三间分布特征和病例免疫史情况。
    结果  2005—2024年累计报告病例2 068例,年均发病率2.19/10万。 2024年病例数激增(1 716例,占82.98%),发病率达39.51/10万。 高发月份由2—7月缩短至4—7月,高发区域接种在六安市金安区、裕安区和叶集区。 5~9岁年龄组儿童逐步取代婴幼儿成为主要发病群体(2024年占比56.29%)。 84.92%病例有白喉破伤风百日咳联合疫苗(DTP)接种史,71.37%完成4剂次,末次接种至发病中位间隔为4年四分位数间距(IQR):3~5年,79.58%为接种后2~6年发病;未接种者中67.69%为未达3月龄的婴儿。
    结论  六安市百日咳2022年起有上升趋势,2024年病例大幅增加,学龄儿童成为主要发病群体。建议通过强化监测体系、提升诊断技术及开展公众健康干预,构建多维度防控策略,遏制百日咳传播。

     

    Abstract:
    Objective  To analyze the epidemiological characteristics of pertussis in Lu'an, Anhui Province, from 2005 to 2024, and provide evidence for the improvement of prevention strategies and evaluation of the effects of immunization program adjustments.
    Methods The data collected from China Disease Prevention and Control Information System and Anhui Provincial Immunization Program Information Management System were analyed with descriptive epidemiological methods to understand the spatiotemporal and population distributions of pertussis cases, and the vaccination history of the cases.
    Results A total of 2 068 pertussis cases were reported in Lu’an during 2005−2024, with an average annual incidence rate of 2.19/100 000. A large number of pertussis cases occurred in 2024 (1 716 cases, 82.98% of the total), with an incidence rate of 39.51/100 000. The incidence peak occurred during April–July instead of during February–July in the past. Jin'an, Yu'an and Yeji were the areas with high incdences. Children aged 5–9 years gradually replaced infants as the mostly affected group, with the case accounting for 56.29% of the total in 2024. In the cases, 84.92% had history of DTP vaccination, in whom 71.37% completed the four doses. The median interval between the last vaccination and disease onset was 4 years interquartile range (IQR): 3–5 years, and 79.58% of the cases occurred 2–6 years later after vaccination. In the unvaccinated cases, 67.69% were infants under 3 months of age.
    Conclusion The incidence of pertussis exhibited an upward trend in Lu'an after 2022, with a high incidence in 2024. School-aged children have become as the mostly affected population. To curb the transmission of pertussis, it is suggested to strengthen the surveillance, improve diagnostic techniques, conduct public health interventions and develop multi-dimensional prevention and control strategy.
