2016年湖北省部分地区手足口病肠道病毒病原谱及柯萨奇病毒A6型和A10型基因特征分析

Pathogen spectrum of hand,foot and mouth disease and genetic characteristics of VP1 of coxsackievirus A6 and A10 in some areas of Hubei,2016

  • 摘要: 目的 了解2016年湖北省部分地区手足口病病原谱特征,对其中的柯萨奇病毒A组6型(Cox A6)和A组10型(Cox A10)基因特征进行分析。方法 采集2016年湖北省部分地区手足口病患者阳性标本,提取病毒核酸,通过一步法反转录-PCR扩增肠道病毒VP1序列并测序,经Blast比对确定病毒基因型。对其中Cox A6和Cox A10的VP1全长序列进行同源性和系统进化分析。结果 病原学监测结果显示,肠道病毒71型(EV71)阳性率为29.4%(599/2 035),柯萨奇病毒A组16型(Cox A16)阳性率为25.7%(522/2 035),其他肠道病毒阳性率为44.9%(914/2 035)。对该地区收集的83份肠道通用阳性的非EV71和非Cox A16进行病毒分型分析,Cox A6阳性检出率为56.6%(47/83),Cox A10阳性检出率为22.9%(19/83),还检测出其他肠道病毒如Cox A2(2株)、Cox A4(7株)、CB4(2株)、CB5(2株)、CB6(1株)、Echo5(1株)、Echo9(1株)和Echo25(1株)。对Cox A6和Cox A10的VP1基因同源性比对及系统进化分析显示,2016年湖北省22株Cox A6的VP1基因核苷酸同源性为95.1%~100.0%,11株Cox A10的VP1基因核苷酸同源性为95.2%~100.0%。我国与其他国家Cox A6和Cox A10可分别划分为8个(A~H)和7个基因谱系(A~G)。湖北省Cox A6与我国其他Cox A6位于H基因谱系上;湖北省Cox A10与我国其他Cox A10位于G基因谱系上。结论 2016年湖北省部分地区手足口病病原体除了EV71和Cox A16两种主要病毒外,还检测出Cox A6和Cox A10等手足口病的病原。其他肠道病毒中以Cox A6和Cox A10为主,仍需进一步加强对其他肠道病毒的监测和分子流行病学特征的分析。

     

    Abstract: Objective To understand the characteristics of pathogen spectrum of hand foot and mouth disease (HFMD) in some areas of Hubei province in 2016 and analyze the genetic characteristics of coxsackievirus A6 and A10.Methods Blood samples were collected from HFMD cases in some areas in Hubei in 2016 for the viral nucleic acid extraction.The full-length VP1 gene of viral RNA was amplified with one-step reverse transcription polymerase chain reaction (RT-PCR) and sequenced.The viral genotypes were identified by the Blast.The homology and phylogenetic analyses of VP1 gene of Cox A6 and Cox A10 were conducted.Results Based on the statistics of etiology surveillance on HFMD in some areas of Hubei in 2016,the positive rates of EV71,Cox A16 and other enteroviruses was 29.4%(599/2 035),25.7%(522/2 035) and 44.9%(914/2 035),respectively.The positive rate of Cox A6 and Cox A10 was 56.6%(47/83) and 22.9%(19/83) respectively in 83 positive samples of non EV71 and non Cox A16.The other enteroviruses detected included Cox A2(2 strains),Cox A4(7 strains),CB4(2 strains),CB5(2 strains),CB6(1 strain),Echo5(1 strain),Echo9(1 strain) and Echo25(1 strain).The 22 strains of Cox A6 shared 95.1%-100.0% nucleotide homology and 11 strains of Cox A10 shared 95.2%-100.0% nucleotide homology respectively.Cox A6 and Cox A10 viruses detected in both China and other countries could be divided into eight (A-H) and seven (A-G) genetic lineages,respectively.Cox A6 viruses detected Hubei and other areas of China were in H lineage.Cox A10 viruses detected in Hubei and other areas of China were in G lineage.Conclusion In addition to EV71 and Cox A16 which were predominant,Cox A6,Cox A10 and other enteroviruses were detected in some areas of Hubei in 2016.Among other enteroviruses,Cox A6 was the main pathogen,followed by Cox A10.It is necessary to strengthen the surveillance for other enteroviruses and related molecular characteristics analysis.
