目的 了解河南省信阳市狮河区人群乙型肝炎(乙肝)病毒(HBV)血清学感染状况.方法 按照全国人群乙肝等有关疾病血清学调查工作手册,采用多阶段随机抽样方法调查,抽取2个乡中的2个行政村,在作主要危险因素的问卷调查的同时,采集对象静脉血,2周岁以上人群4 ml,满12月龄但不足2周岁者2 ml,进行血清分离,送中国疾病预防控制中心进行HBV血清学指标检测.结果 共调查547人,其中男性275人,女性272人,检出乙肝表面抗原(HBsAg)阳性36例,阳性率6.58%,表面抗体阳性390人,阳性率71.30%,乙肝病毒核心抗体(抗-HBc)阳性207例,阳性率37.84%,HBV 感染者208例,感染率38.02%,标化阳性率分别为7.98%、60.73%、42.95%、42.94%.乙肝病毒HBsAg及抗-HBc男女之间差异无统计学意义,抗-HBc和HBV感染男女之间差异有统计学意义.结论 信阳市狮河区HBV感染率已从1992年的72.62%(标化后)降至42.94%(标化后),HBsAg标化率由11.04%降至7.98%.显示出推广乙肝疫苗接种效果.
Objective The study was conducted to serologically investigate the hepatitis B virus (HBV) infections in Shihe River district of Xinyang city in Henan province. Methods A multi-stage randomized sampling survey was conducted in accordance with the National Manual for Serological Investigation of Hepatitis B and Other Related Diseases, involving two administrative villages from two townships. Apart from the questionnaire surveys on major risk factors, intravenous blood samples were collected for HBV serological detection in the national CDC.(4 ml for each person older than 2, 2ml for each older than 12 months and before age 2). Results A total of 36 hepatitis B surface antigen-positive cases were detected from 547 investigated subjects including 275 men, 272 women, the positive rate being 6.58%; 390 surface antibody-positive cases were detected, the positive rate being 71.30%; and 207 core antibody positive cases were found, the positive rate being 37.84%. There were 208 hepatitis B virus infected people, the infection rate being 38.02%; the standardized positive rates were 7.98%, 60.73%, 42.95%, 42.94%, respectively. There was no statistical significance on HBsAg and anti-HBc levels between men and women, although the difference on anti-HBc and HBV infections between both presented statistical significance. Conclusion The HBV infection rate in Shihe River district of Xinyang city has declined from 72.62%(post-standardized) in 1992 to 42.94%(post-standardized), the HBsAg standardized rate from 11.04% to 7.98%, indicating the contributing effect of promoted hepatitis B vaccination.
