Spatial Temporal clustering of tuberculosis in students in Chongqing, 2014–2019

      目的   分析重庆市2014 — 2019年学生结核病登记病例的时空分布特征和流行趋势,为学生结核病防控策略提供依据。
      方法   利用“中国疾病预防控制信息系统”收集2014 — 2019年重庆市学生结核病登记病例数据,应用地理信息软件ArcGIS 10.3进行全局及局部空间自相关分析,应用SaTScan 9.6软件探测病例的时空聚集性。
      结果   2014 — 2019年重庆市学生结核病年均登记率为30.23/10万,总体呈现缓慢下降趋势,男性年均登记率高于女性,差异有统计学意义(χ2=82.336,P<0.001),高中教育阶段年均登记率最高(81.05/10万)。 历年全局空间自相关分析显示不同地区学生结核病登记率呈正相关且具有统计学意义(P<0.05)。 局部自相关分析显示学生结核病登记率呈高–高值聚集和低–低值聚集且具有统计学意义(P<0.05)。 时空聚集性分析扫描出历年来最大可能聚集区主要位于渝东南片区(P<0.05),聚集时间逐年由上半年转向下半年。
      结论   渝东南片区是重庆市学生结核病高负担聚集区,同时也是传播高风险聚集地区,研究结果可为区域学生结核病防控策略的制定提供参考。

     

    Abstract:
      Objective  To analyze the epidemiological characteristics and spatial-temporal distribution of registered tuberculosis (TB) cases in students in Chongqing from 2014 to 2019 and provide evidence for TB prevention and control.
      Methods  The data of registered TB cases in students in Chongqing from 2014 to 2019 were collected from national information system for disease prevention and control. Software arcgis10.3 was used for global and local spatial autocorrelation analyses and software SaTScan 9.6 was used to detect the spatiotemporal clustering of TB cases.
      Results  The average annual registration rate of TB in students in Chongqing was 30.23/100000 during 2014−2019, showing a downward trend. The annual TB registration rate in men was significantly higher than that in women, the difference was significant(χ2=82.336, P<0.001). The average annual registration rate in senior high school was highest (81.05/100000). The spatial correlation analysis showed that the annual registration rate of TB in students in different areas had positive spatial correlation with significance (P<0.05). Local autocorrelation analysis showed that the TB registration rate in students showed high-high and low-low clustering, the difference was significant (P<0.05). The results of spatial-temporal scanning showed that the annual highly possible clustering area was in southeastern Chongqing (P<0.05). The clustering time has gradually shifted from the first half of a year to the second half of a year.
      Conclusion  Southeastern Chongqing has the highest burden of TB in students and is a high-risk clustering area. These findings can be used as evidence for the development intervention strategies for TB prevention and control in students in Chongqing.
