2021年江苏省苏州市1~3岁幼儿屈光异常与母亲孕期疾病的关联研究

Relationship between pregnant diseases and refractive errors among children aged 1–3 years old in Suzhou, Jiangsu province, 2021

    目的 分析江苏省苏州市1~3岁幼儿屈光异常与母亲孕期疾病的关联。
    方法 采用整群抽样方法,将2021年1—12月苏州市高新区各社区卫生服务中心进行体检的1~3岁幼儿为研究对象。 通过问卷调查和眼科检查,收集研究对象的基本人口统计学特征、围产期相关因素、家庭环境因素和裂隙灯检查、早产儿视网膜病变筛查、屈光筛查及眼球运动检查等信息。 采用logistic回归模型对幼儿屈光发育与母亲孕期疾病的关系进行分析。
    结果  8 364名研究对象,平均年龄为(24.65±3.38)月龄,屈光异常率为47.80%,其中散光率、疑似屈光参差率、疑似近视率和异常远视率分别为44.50%、1.70%、1.00%和0.60%。 多因素logistic回归模型分析结果显示,调整混杂因素后,母亲孕期患有高血压、甲状腺和感染性疾病等的幼儿屈光异常发生风险较高[比值比(95%置信区间)分别为2.05(1.49~2.82)、1.50(1.27~1.77)和2.09(1.66~2.63)]。
    结论 母亲孕期所患疾病与幼儿屈光异常有关联。

     

    Abstract:
    Objective The purpose of the study was to analyze the relationship between pregnant diseases and refractive errors of children aged 1–3 years old in Suzhou, Jiangsu.
    Methods A total of 8364 children aged 1–3 years old who went to the community health centers in Suzhou High-tech Zone for physical examinations were included in the data analysis from January to December, 2021. Information regarding demographic factors, perinatal factors and lifestyle-related exposures were collected by per-designed questionnaires. The ophthalmic examinations including slit lamp inspection, retinopathy of prematurity screening, auto-refraction and ocular motility examination were conducted. Logistic regression models were fitted to estimate the relationship between pregnant diseases and refractive errors.
    Results In this study, the total rate of refractive errors were 47.80%. The prevalence of astigmatism, anisometropia, myopia and abnormal hyperopia among 8364 Suzhou children (mean±standard deviation: 24.65±3.38 months) was 44.50%, 1.70%, 1.00% and 0.60%, respectively. In multivariate analysis, the occurrence of refractive errors was positively associated with hypertensive disorder complicating pregnancy odd ratio (OR)=2.05, 95% confidence interval (CI): 1.49–2.82, thyroid disease (OR=1.50, 95%CI: 1.27–1.77) and infectious diseases (OR=2.09, 95%CI : 1.66–2.63) (all P<0.05).
    Conclusion The presence of refractive errors was related to the pregnant diseases of children in Suzhou.
