2017-2022年浙江省湖州市食源性腹泻病例中副溶血弧菌血清型分析

Analysis on serotypes of Vibrio parahaemolyticus isolated from foodborne diarrhea cases in Huzhou, Zhejiang, 2017−2022

    目的 了解浙江省湖州市食源性腹泻病例中副溶血弧菌菌株的血清型别及分子分型特征,为预防食源性疾病提供科学依据。
    方法 对分离自腹泻患者的副溶血弧菌菌株开展血清学分型、毒力基因检测,采用多位点序列分型(MLST)分析菌株之间的遗传关系。
    结果 2017—2022年湖州市食源性疾病腹泻病例分离688株副溶血弧菌,共鉴定出33个血清型,其中以O3:K6(48.84%)、O10:K4(16.72%)和O4:KUT(12.79%)为优势型别。 毒力基因型别以tlh+ tdh+ trh-为主(96.51%);核心基因MLST分析显示O3:K6和O10:K4各自聚成一支,O4:KUT分成3部分,明显分成2支的分别是2020年和2022年分离的菌株,另外还有2株2020年的O4:KUT和O3:K6菌株聚在一起。 序列分析表明,69株副溶血弧菌有8种耐药相关基因,涉及4种抗生素类别。
    结论 湖州市副溶血弧菌优势血清型别已经发生明显变化。 应持续进行副溶血弧菌血清型及分子特征监测,关注近几年检出率上升的血清型O10∶K4和O4:KUT引起大流行的可能性。

     

    Abstract:
    Objective To understand the serotype and molecular characteristics of Vibrio parahaemolyticus strains isolated from foodborne diarrhea cases in Huzhou, Zhejiang province, and provide evidence for the prevention and control of foodborne diseases.
    Methods Serological typing, virulence gene testing, and multilocus sequence typing (MLST) were performed for the V. parahaemolyticus strains isolated from diarrhea cases.
    Results From 2017 to 2022, a total of 688 strains of V. parahaemolyticus were isolated from foodborne diarrhea cases in Huzhou, and 33 serotypes were identified, in which O3: K6 (48.84%), O10: K4 (16.72%), and O4: KUT (12.79%) were predominant. The main virulence genotype was tlh+tdh+trh- (96.51%). Serotypes O3: K6 and O10: K4 all belonged to ST3, while serotype O4: KUT was divided into 3 STs, ST3, ST332 and ST2516/265. Core-genome MLST clustering analysis showed that O3:K6 and O10:K4 were basically clustered together. O4: KUT was clearly divided into three parts, and only the strains isolated in 2020 and 2022 could be divided into 2 clades. Sequence analysis revealed that 69 strains of V. parahaemolyticus carried 8 resistance related genes, involving 4 antibiotic categories.
    Conclusion The predominant serotypes of V. parahaemolyticus in Huzhou had shifted obviously. It is necessary to carry out consecutive surveillance for serotypes and molecular characteristics of V. parahaemolyticus and pay attention to the possibility of a pandemic caused by serotypes O10: K4 and O4: KUT with increased detection in recent years.
