Epidemiological characteristics of human brucellosis in Tongzhou district of Beijing, 2018−2020

      目的  分析2018—2020年北京市通州区人间布鲁氏菌病(布病)的流行病学特征,探索人群布病感染来源,为通州区布病的防治工作提供数据支持。
      方法  对2018—2020年通州区报告布病病例流行特征、感染来源、防护情况、发病至确诊时间间隔、病原学检测及临床表现进行统计分析。 采用χ2检验对各年度和不同性别发病率进行差异性比较,P<0.05为差异有统计学意义。
      结果   2018—2020年通州区累计报告布病病例53例,发病率逐年降低;春夏季高发,高峰期集中在3—9月;发病地区以农村及城乡结合部为主;男性平均发病率为1.82/10万,女性为0.39/10万,性别差异有统计学意义(χ2=21.67,P<0.001);发病年龄段以40~69岁最高(83.02%);职业分布以农民为主;感染来源主要以接触羊、牛等牲畜及其制品为主;仅有15.79%的病例在饲养等过程中采取了有效防护;56.60% 的病例能够在1个月以内确诊。
      结论   2018—2020年通州区布病发病率逐年下降,发病呈现出明显的季节性和地区性差异。 发病以男性、农民为主,40~69岁为高发年龄段。 应根据流行病学特征,有针对性的采取防控措施。

     

    Abstract:
      Objective   To understand the epidemiological characteristics of human brucellosis in Tongzhou district of Beijing from 2018 to 2020, identify the sources of human brucellosis infection and provide data support for the prevention and treatment of human brucellosis in Tongzhou.
      Methods   Statistical analysis was conducted on the epidemiological characteristics, sources of infection, protection measures, time interval from onset to diagnosis, etiological detection and clinical manifestations of reported brucellosis cases in Tongzhou from 2018 to 2020. The differences in incidence of human brucellosis among different years and between men and women were compared with χ2 test, and P<0.05 was considered statistically significant.
      Results   From 2018 to 2020, a total of 53 confirmed cases of human brucellosis were reported in Tongzhou, and the overall incidence declined by year. The annual incidence was high in spring and summer with the peak during March - September. The cases mainly occurred in rural areas and urban-rural continuum. The incidence rate was 1.82/100 000 in men and 0.39/100 000 in women, the difference was significant (χ2=21.67, P<0.001). The incidence was higher in age group 40-69 years (83.02%), and the cases were mainly farmers. The main source of infection was contacts with sheep, cattle and their products, and only 15.79% of the cases took effective protection measures in the process of feeding of animals. In the reported cases, 56.60% were diagnosed within one month.
      Conclusion   From 2018 to 2020, the overall incidence of human brucellosis in Tongzhou showed a decline trend. The obvious seasonal and area differences were observed. The cases were mainly men, farmers and aged 40−69 years. The targeted prevention and control measures should be taken according to the epidemiological characteristics of human brucellosis.
